LE LOEB <br /> ELECTRIC <br /> ■ Quantity of Arc Flash Relays (indoor and outdoor) and infrared windows <br /> requested <br /> ■ Provide a detailed description and price breakdown of any items or services not <br /> covered by the Scope of Work (i.e. additional tasks needing completed in order <br /> to perform the AFRI tasks detailed in this Scope of Work document). <br /> • Submit to site team and escalate to EE Team. <br /> ■ Date and time of outage <br /> • The following assumptions shall be made when quoting outages <br /> (reference 1.3 "Access Hours"for additional details). <br /> o Sites with 1-2 main breaker lineups shall reflect a 12-hour outage <br /> o Sites with 3-4 main breaker lineups shall reflect a 14-hour outage <br /> o Sites with 5 or more main breaker lineups will be discussed prior <br /> to proposal submission <br /> If it is identified during the pre-planning walkthrough that Amazon or landlord possesses <br /> ownership of the upstream transformers or generator,the VEEC is required to provide a proposal <br /> to perform maintenance on said equipment and any MV equipment.If this is beyond the VEEC's <br /> capabilities, the VEEC must inform Loeb so other accommodations can be made. If the <br /> transformer is an oil-filled padmount, the proposal should include the cost to purchase and <br /> install oil sample plumb out kit (see Appendix Q). An oil sample is to be pulled on the day of <br /> outage on any oil filled equipment and sent to SDMeyers lab. Results must be added to the PM <br /> Report. <br /> • Punch List/Change Orders: All punch list and change order items are required to be reported <br /> to Loeb and documented in a written email within (6) hours of discovery. Following discovery, <br /> approval of the requested change order will be required before any pricing is submitted for <br /> review. If the request to submit a change order is approved, pricing is due within (3) days of <br /> approval. Change orders will only be considered if submitted within (24) hours and the cause of <br /> change is unavoidable. <br /> • Rescheduling: Contracted parties are not permitted to alter schedule dates unless approved by <br /> the Loeb Project Manager. <br /> • RFI's: Loeb Electric expects the VEEC and Electrical Contractor to do their own due diligence in <br /> distinguishing and identifying any install uncertainties, design discrepancies/errors, any ambiguity or <br /> concern regarding site conditions, etc. All findings must be reported to Loeb Electric within (48) hours of <br /> discovery. <br /> • Safety: Contracted party employees are required to follow all applicable NFPA 70E standards, <br /> OSHA, federal, state, or local safety requirements. Employees must be current on CPR, and <br /> OSHA 1910.32 (a), 1910.332 (b), and 1910.269 certifications and standards.Work will not be <br /> performed in situations where the proper level of PPE is not practical. <br /> • Safety Huddle:The VEEC Team Lead will be responsible for leading a huddle at the beginning <br /> of the outage.It is expected that all parties on site (VEEC, EC,Amazon) will attend the huddle. <br /> Huddles should be kept under 15 minutes and should include topics of who is charge of the <br /> crews at each station, how communication will flow from the individual crews to the Team Lead, <br /> distribution of radios, what utility de-energization and re-energization will look like, timing <br /> expectations, etc. <br /> • Schedule: Contractors will need to adhere to a schedule that will be pre-determined based on <br /> Amazon's priorities. Weekly schedule updates are a baseline requirement. During outages the <br /> schedule reporting requirements will be met through the use of the External Partner <br /> Submission Form. <br /> • Scope of Work Declaration: Instruction provided within this Scope of Work document <br /> supersedes any other Scope of Work documents, either past or present.
