LE LOEB <br /> ELECTRIC <br /> 1.3 Required Program Deliverables <br /> Work will only be considered complete when all required deliverables are received. Deliverables must be submitted <br /> through the Amazon External Partner Submission Form, when applicable. <br /> • Pre-Planning: <br /> o Site Access Request <br /> ■ At a minimum of(2) business days prior to any site visit, a "Site Access Request" <br /> must be filled out by any persons that will be on site. <br /> • Information Needed Includes: <br /> o Company Type (Vendor or EC) <br /> o Company Name <br /> o Technician Full Legal Name, DOB, and Contact Information <br /> o Proposed Site Access Date/Time <br /> o Amazon Orientation form (yearly) is completed. <br /> ■ See Appendix C for further information <br /> o Copies of any applicable permits <br /> o Written LOTO and switching plan <br /> o Scope review call completed with Amazon EE <br /> o Review and proposal for IR deficiency repairs <br /> o Material pre-checks at VEEC shop <br /> • Outage: <br /> o The VEEC (s) will designate a singular point-of-contact prior to the scheduled outage.It <br /> will be the responsibility of this individual to provide updates to the Loeb Project <br /> Manager using "Outage Site Updates". <br /> o Leave a printed copy of correct relay SOP with each relay. See Appendix V for latest copy <br /> of SOP. <br /> • Post-Outage: <br /> o "RMA/Material Return Request" for any unused IR Windows. <br /> o Any work found during the AFRI and PM process that could not be addressed during <br /> outages must be quoted within (5) business days of the outage.If the VEEC is unable to <br /> provide a quote within (5) business days, the VEEC is required to inform Loeb so other <br /> accommodation can be made. <br /> o The completed PM report is due to Loeb within (15) business days of the <br /> completed PM. See Appendix U for required template. <br /> • Report must be submitted through "Report Submission" <br /> ■ Provide a redline of Arc Flash Relay schematic if any changes were made to how <br /> the AF Relay was installed <br /> ■ Include pre-outage enclosure test checklist and results <br /> • Include Outage execution checklist and PowerDB results <br /> ■ Include as left .rdb file <br /> • Include pictures taken as part of scope walk, pre-checks and day of outage <br /> 11 � <br />
