LE LOEB <br /> ELECTRIC <br /> • Equipment: If the use of a client owned asset is required to execute a portion of the work <br /> needed, please notify the Loeb Project Manager in advance so that accommodations can be made <br /> with the site prior to job execution. <br /> • Findings: As part of the Arc Flash Relay Install Program, all parties involved have a moral <br /> obligation to call out any finding and/or concerns. As a rule, the best practice to follow is "if you <br /> see something, say something".This includes calling out situations where instruction in this scope <br /> document cannot be followed as outlined and additional adjustments need made. <br /> • Hot Work: Equipment/Circuits must be de-energized during all PM services, IR window <br /> installs, and arc flash relay tie-in. Preparatory work, such as mounting and conduit runs, can be <br /> completed if no hot work is being performed. <br /> o Any Contractor found to be performing "hot"work is subject to immediate removal <br /> from the program. Amazon associate non-compliance of de-energized circuits should <br /> be reported to the assigned Loeb Project Manager immediately. <br /> • Invoicing:All invoicing must be submitted to Loeb within (7) business days of task completion. <br /> Invoice line items must match purchase order line items. Loeb Electric's payment terms are 60 <br /> days. <br /> o All invoices must be sent to AcctsPay@LoebElectric.com, cc <br /> AMZProjects@loebelectric.com <br /> o Invoices must match Proposal,variations to the original proposal need to be made <br /> through a change order request. <br /> o PRM and AFRIP scope must be on separate invoices. <br /> o All invoices need to have the following <br /> • Site ID# <br /> ■ Loeb purchase order number <br /> ■ Date of execution <br /> • Job Safety Analysis (JSA)/Job Hazard Analysis (JHA): All contracted employees are required to <br /> submit a JSA to Loeb prior to site visits. In addition, a hard-copy JSA/JHA must be taken to each <br /> site and submitted to Amazon, if requested. A JSA/JHA template can be found in Appendix D. <br /> • Key Performance Indicators and Deliverables: All deliverables are required to be submitted through <br /> single source form in Appendix E. If submitted data needs corrected, please notify your Loeb Project <br /> Manager and they will assist in correcting or modifying information. KPI's will be tracked throughout this <br /> program in accordance to the below tables, (EC table, VEEC table respectively). The intent of these KPI's is <br /> to measure the performance of the contractors on current work and award additional work based on <br /> results. Removing awarded work is not the intent of the KPI performance. KPI Metrics can be found in <br /> Appendix W. <br /> • Lockout Tagout (LOTO) Calls: The purpose of the LOTO calls is to review, discuss and agree <br /> on the best plan being provided by the VEEC of where electrical equipment will be safely <br /> locked out. This plan is to ensure the main electrical equipment is properly de-energized and <br /> to ensure the safety of all participants in this program. The VEEC will provide a one-line mark- <br /> up drawing no later than 2 weeks prior to the sites execution date. Once the one-line is <br /> received by Loeb, a LOTO call will be scheduled and the following will be required to attend: <br /> Amazon Onsite Technical Lead,VEEC Onsite Engineer, Loeb LOTO Project Manager, EC Onsite <br /> Technical Lead and Utility Representative. <br /> 61 Page <br />
