<br />Lexington-Broadway Stormwater Imp. Phase 1 22 Scope of Services <br />2. Prepare and submit Ecology Section 401 WQC pre-filing request and 401 request form to <br />initiate the permit. <br />3. Prepare and submit the following Phase 1 permit application materials: <br />A. City of Everett Generic Land Use Permit Application Package: <br />i. Land Use Application. <br />ii. Site Plan that abides by the “Plan Set Packet Checklist” provided by City of Everett <br />Planning Division, most recently updated in 2021. <br />iii. Narrative statement. <br />iv. Mailing list (to be provided by City) <br />v. SEPA checklist (prepared above, under Task 1D). <br />B. Notice of Intent (NOI) for use of NPDES Construction Stormwater General Permit. <br />4. Prepare and submit the following Phase 2 permit application materials: <br />A. City of Everett Application for Land Use Permit, Shoreline Substantial Development <br />Permit, and Critical Areas Review, through the One Project Review Process, of EMC <br />15.02.050, to include the following: <br />i. Preapplication meeting notes. <br />ii. Land Use Application. <br />iii. Site Plan that abides by the “Plan Set Packet Checklist” provided by City of Everett <br />Planning Division, most recently updated in 2021. <br />iv. SEPA checklist (prepared above, under Task 1D). <br />v. JARPA (prepared above, under Task 1E). <br />vi. Mailing list (to be provided by the City). <br />vii. Special Studies: BA, CAR, Geotechnical Engineering Report. <br />B. Floodplain development permit application consistent with requirements in EMC <br />19.30.080A(2). <br />C. Submit JARPA and supporting documentation to USACE to initiate Federal Permit <br />Review of Phase 2. <br />5. Provide agency coordination services during permit review. This task will include application <br />for, attendance at, and project support during one USACE pre-application meeting. This task <br />will also include attendance at other virtual meetings, and phone calls, and up to 3 on-site <br />meetings. This task also includes one round of edits to permit application materials and <br />supporting documents following agency review of application materials. Support will be <br />limited to 130 staff hours. Additional support may be provided subject to a change order. <br />City Responsibilities <br />The City is responsible for the following activities:
