For each and 31 every DBE firm listed on the Bidder’s completed Disadvantaged Business Enterprise 32 Utilization Certification, the Bidder shall submit written confirmation from that DBE firm 33 that the DBE is in agreement with the DBE participation commitment that the Bidder has 34 made in the Bidder’s completed Disadvantaged Business Enterprise Utilization 35 Certification. 36 37 WSDOT Form 422 031 (Disadvantaged Business Enterprise Written Confirmation 38 Document) is to be used for this purpose. Bidder must submit good faith effort 39 documentation only in the event the bidder’s efforts to solicit sufficient DBE participation 40 have been unsuccessful. 41 42 The Bidder shall submit a DBE Bid Item Breakdown form defining the scope of work to 43 be performed by each DBE listed on the DBE Utilization Certification. 44 45 If the Bidder lists a DBE Trucking firm on the DBE Utilization Certification, then the Bidder 46 must also submit a DBE Trucking Credit Form (WSDOT Form 272-058) documenting how 47 the DBE Trucking firm will be able to perform the scope of work subcontracted to them. 48 49 Directions for delivery of the Disadvantaged Business Enterprise Written Confirmation 50 Documents, Disadvantaged Business Enterprise Good Faith Effort documentation, DBE 51
