preconstruction photographs and video, developing a Schedule of Values, and submitting the initial <br />project schedule. Also includes costs for all subcontracted work. Also include demobilization and <br />removal of all temporary improvements upon completion of the project. <br />Develop and execute the project specific Health and Safety Plan (HASP) and provide required <br />training. This includes, but is not limited to: <br />(a) Coordination, preparation, submittal, implementation, maintenance and updates to the site - <br />specific Health and Safety Plan (HASP). <br />(b) Execution of the HASP including a qualified health and safety representative. <br />(c) Compliance by the Contractor and subcontractors to implement the HASP for the duration of <br />this Contract. <br />(d) Coordination with the City, the City's Representative, the Engineer, and jurisdictional and/or <br />regulatory agencies regarding health and safety matters. <br />(e) Furnishing, maintaining, and compliance on the use of safety and protective equipment and <br />facilities. <br />(f) Documentation, records and reports for compliance with the HASP and regulatory <br />requirements. <br />Items not included in this item also include, but are not limited to: <br />(a) Work covered by a specific bid item or Work that is to be included in a bid item or items. <br />(b) Profit, interest on borrowed money, overhead or management costs. <br />Partial payments will be made for the lump sum contract price for "Mobilization" as follows: <br />(a) When 5% of the total original contract amount is earned from other bid items, 50% of the <br />amount bid for mobilization, or 5% of the total original contract amount, whichever is the least, <br />will be paid. <br />(b) When 10% of the total original contract amount is earned from other bid items. 50% of the <br />amount bid for this bid item, or 5% of the total contract amount, whichever is the least, will be <br />paid <br />(c) The remaining amount will be paid after demobilization is complete. <br />Upon completion of all work on the project, including demobilization, payment of any amount bid <br />for mobilization in excess of 10% of the total original contract amount will be paid. <br />Bid Item 2 - SURVEYING AND CONSTRUCTION STAKING <br />Measurement and Payment: Lump Sum (LS) <br />The lump sum bid price for this bid item shall be full compensation for all submittals, furnishing all <br />material, equipment, tools, and labor necessary for surveying and construction staking. <br />This bid item includes coordination with the City of Everett for locating and establishing project <br />control points and benchmarks, for setting and monitoring the location, elevation, alignment and <br />grade of the Work as specified in WSDOT Standard Specification Section 1-05.4 and the <br />specifications and Drawings. <br />This bid item also includes establishing and staking of the proposed facilities, utility improvements <br />and other easement lines, public right of way lines, and new fences, as required, to complete the <br />work. This includes all work necessary to provide construction survey, control monuments, <br />temporary benchmarks, roadways, walls, boundaries, and other related surveying and staking <br />necessary for construction. <br />The lump sum bid price for this bid item also includes the work necessary for potholing and verifying <br />pipe connection points to existing pipes and structures and pipe grades and elevations, with the <br />potholing occurring prior to construction. (Does not include potholing for items listed in any <br />excavation and other bid items.) <br />The lump sum bid price for this bid item also includes for obtaining the information necessary for <br />the "as constructed" location and elevations of the Work, in particular reservoir dimensions and <br />pipe invert elevations and other information necessary for production of the Record Drawing (As - <br />Constructed) documents. <br />This bid item does not include preparation of the Record Drawings as this is a separate bid item. <br />This bid item does not include surveying for settlement monitoring. <br />Reservoir 3 Replacement Phase 1 <br />Work Order No. UP3739 01 22 00 <br />MEASUREMENT AND PAYMENT March 2024 <br />
