CHANGE ORDER # <br />Project (Procurement) Number: Small Works Roster Project Title: <br />LSWR: 2024-059 Water Filtration Plant Gutter Replacement <br />Description of Change: (Attach contractor change proposal and cost schedule) (Attach Price or Cost Analysis) <br />BUILDING/LOCATION: 6133 Lake Chaplain Road, Monroe, WA 98272 <br />This change order replaces Filtration Plant Aluminum Poly Building. <br />During the initial inspection by city staff of the Aluminum Poly Building, the awning extending over the <br />stair landing was identified as the source of a water leak. Staff obtained quotes from qualified <br />suppliers to correct the leaking awning. <br />After the contract was awarded and the contractor completed repairs on the aluminum poly building <br />awning, water continued to leak from the building.
