Construction documents referenced for today's
inspection include Conformed plan set dated 06/18/21. Earthwork was performed by Fabor Construction.
Native material exposed in northwest corner was grey and brown silty sand with gravel. We probed the entire
area with a 1/2" diameter T-handled probe rod with minimal to no penetration. It is our opinion that the area
probed is firm and unyielding, sufficient to support desired loads.
Native material exposed in the southeast corner was brown silty sand with trace gravel. We identified a 3' by 6'
area in the southeast corner as saturated and unconsolidated. We probed this 3' by 6' area with a 1/2" diameter
T-handled probe rod with severe penetration. Contractor removed saturated material to depth of 2', placed filter
fabric and quarry spalls, followed by a second layer of filter fabric and 1-1/4" crushed surface and base course.
We probed the surrounding area in the southeast corner with T-handled probe rod with minimal to no penetration.
The native material exposed surrounding the 3' by 6' area appears firm and unyielding, sufficient to support
desired loads.
Native material below trench drain removal was brown silty sand with trace gravel. We probed this entire area
with 1/2" diameter T-handled probe rod with minimal penetration. It is our opinion the area below previous trench
drain is firm and unyielding, sufficient to support desired loads.
Results of today's inspection were reported to Forma Construction.
COMPLIANCE STATEMENT
Based on our observations and testing, the above noted items appear to be in general accordance with the
project documents.
Services: Special Inspections/ Materials Testing
Mayes/Terracon Rep.: Stephen Johnson
Reported To: Vanir Construction Services
Contractor: Vanir Construction
Report Distribution:
(1)Vanir Construction Management (1)City of Everett WA,Kurt Keck
Inc.,John Wriqht
(1)Community Transit,Larry (1)Jimale Technical Services LLC, Reviewed By: lb
Daubenmire Tom Donnelly --
(1)Mayes Testing Engineers,Inc, (1)Mayes Testing Engineers,Inc, Barry Tuttle
Donna Pollard Todd Wirtz
(1)Vanir Construction Management (1)Vanir Construction Management Project Field Manager
Inc.,Dan McNay Inc.,John Turk
The tests were performed in general accordance with applicable ASTM,AASHTO,or DOT test methods.
