Asbestos and Lead Inspection <br />Everett Parks and Facilities Department | Everett, Washington <br />July 2, 2024 | Terracon Proposal No. P81247400 <br />Facilities | Environmental | Geotechnical | Materials 3 <br /> <br /> <br /> <br />prior to incurring additional project costs. If authorized, additional samples will be billed at <br />$25.00 per sample. <br /> <br />The paint chip samples will be submitted to a laboratory participating in the Environmental <br />Lead Laboratory Accreditation Program (ELLAP) for lead content analysis on a 5-Day TAT. <br />Limited LCP sampling will result in damage to a small area (approximately 2 square inches) <br />of the painted surface at each sample location. Terracon will not be responsible for <br />repainting sampled surfaces. <br />2.3 Report Preparation <br />Terracon will prepare a written report that includes the following: <br /> <br /> Project description and scope of work. <br /> <br /> Summary of findings. <br /> <br /> Sampling, inspection, and evaluation methods. <br /> <br /> Results of the ACM and LCP inspection. <br /> <br /> The number, type and location of samples collected for analysis. <br /> <br /> Laboratory analytical results for samples collected. <br /> <br /> Photographs of the suspect ACM sampled. <br /> <br /> Estimated quantity of materials identified as ACM. <br /> <br /> Sample location diagrams. <br /> <br /> Data evaluation and discussion with conclusions and recommendations for further <br />action, if necessary. <br />Unless otherwise instructed, one PDF-formatted copy of the final report will be submitted to <br />the Client via email. <br />2.4 Reliance <br />Any documents resulting from this project will be prepared for the exclusive use and <br />reliance of the Client. Reliance by any other party is prohibited without the written <br />authorization of the Client and Terracon. If the Client is aware of additional parties that will <br />require reliance on the documents, the names, addresses and relationship of these parties <br />should be provided for Terracon approval prior to the time of authorization to proceed. <br />Terracon will grant reliance on the documents to those approved parties upon receipt of a <br />fully executed Reliance Agreement (available upon request) and receipt of an additional fee <br />of $450.00 per relying party.
