Exhibit A <br />HCS-24-66-2440-198 <br />City of Everett <br />Page 10 of 15 <br />Permanent housing receiving funding under this Contract must meet the <br />habitability standards in accordance with Consolidated Homeless Grant <br />(CHG)/System Demonstration Grant (SDG) guidelines. <br /> <br />C. Coordinated Entry System <br /> <br />The Agency shall use the Coordinated Entry system established by the <br />Everett/Snohomish County Continuum of Care as required by the Department <br />of Housing and Urban Development (HUD) CoC Program Coordinated Entry <br />Requirements and guidelines as described in the Consolidated Homeless <br />Grant (CHG)/System Demonstration Grant (SDG) guidelines. <br /> <br />Transitional Housing, Homelessness Prevention, Rapid Re-Housing, and <br />Permanent Supportive Housing projects funded by EHF must participate in <br />Snohomish County’s Coordinated Entry (CE) by accepting referrals and must <br />fill openings exclusively through the CE process. <br /> <br />XII. RECORDS <br /> <br />A. Records to Be Maintained <br /> <br />The Agency shall maintain all records pertaining to the activities funded under <br />this Contract and as further described in Exhibit B and shall furnish such <br />records to the County, Commerce, or other authorized Federal officials, as <br />requested. The Agency shall maintain records including, but not limited to: <br /> <br />1. Books, records, documents, data and other evidence relating to this <br />contract and performance of the services described herein, including but <br />not limited to accounting procedures and practices that sufficiently and <br />properly reflect all direct and indirect costs of any nature expended in the <br />performance of this contract. <br /> <br />2. The above records and all other financial records, supporting documents, <br />statistical records, and all other records pertinent to this Contract shall be <br />retained for a period of six (6) years following the date of final payment or <br />termination of this Contract. At no additional cost, these records, including <br />materials generated under the contract, shall be subject at all reasonable <br />times, to inspection, review or audit by Commerce, personnel duly <br />authorized by Commerce, the Office of the State Auditor, and federal, state <br />and county officials so authorized by law, regulation or agreement. <br /> <br />If any litigation, claim or audit is started before the expiration of the six (6) year <br />period, the records shall be retained until all litigation, claims, or audit findings <br />involving the records have been resolved. <br /> <br />
