Participant Information and Confidentiality <br /> <br />The Agency understands that participant information collected under this <br />Contract is private and the use or disclosure of such information, when not <br />directly connected with the administration of the County’s or Agency’s <br />responsibilities with respect to services provided under this Contract, may be <br />prohibited by federal, State, and local laws regarding privacy and obligations of <br />confidentiality, unless written consent is obtained from such person receiving <br />service and, in the case of a minor, that of a responsible parent or guardian. <br /> <br />XIII. PERFORMANCE REVIEW <br /> <br />At a minimum, the County shall review quarterly the Agency's level of planned <br />performance compared to actual performance as reported on monthly expenditure <br />and reports as provided pursuant to Exhibit B of this Contract. If the Agency has <br />expenditure issues, or performance issues related to utilization or outcomes <br />defined in Exhibit B, the County may reallocate those funds to other eligible <br />program agencies. Any reduction will be based on actual performance. The level <br />of funding reduction shall be negotiated between the Agency and the County, with <br />the County retaining the authority to set the reduction level. <br /> <br />XIV. PROHIBITIONS <br /> <br />Funds awarded under this Contract shall not supplant other existing funding <br />sources. Funds awarded under this Contract shall not be used for: <br /> <br />A. Activities not related to the EHF Program; <br /> <br />B. Administrative expenses above the amount specified in Exhibit C; and <br /> <br />C. Non-expendable equipment with a cost exceeding $1,000.00 per item, without <br />prior approval of the County. <br /> <br />XV. HOMELESS MANAGEMENT INFORMATION SYSTEM PARTICIPATION <br /> <br />A. The Agency shall participate in the Snohomish County Homeless Management <br />Information System (“Snohomish County HMIS”) which is administered by the <br />County’s Human Services Department. Snohomish County HMIS is an <br />electronic database that collects data on homeless persons who receive <br />coordinated entry, outreach, homeless prevention and intervention services, <br />emergency shelter, transitional housing, supportive services not linked with <br />housing, and permanent housing, including rapid rehousing, permanent <br />supportive housing, and other permanent housing. <br /> <br />B.
