It will 8 <br />also list estimated quantities, units of measurement, the items of work, and the materials 9 <br />to be furnished at the unit bid prices. The bidder shall complete spaces on the proposal 10 <br />form that call for, but are not limited to, unit prices; extensions; summations; the total bid 11 <br />amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment 12 <br />of addenda; the bidder’s name, address, telephone number, and signature; the bidder’s 13 <br />UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor’s 14 <br />Registration Number; and a Business License Number, if applicable. Bids shall be 15 <br />completed by typing or shall be printed in ink by hand, preferably in black ink. The 16 <br />required certifications are included as part of the Proposal Form. 17 <br /> 18 <br />The Contracting Agency reserves the right to arrange the proposal forms with alternates 19 <br />and additives, if such be to the advantage of the Contracting Agency. The bidder shall 20 <br />bid on all alternates and additives set forth in the Proposal Form unless otherwise 21 <br />specified. 22 <br /> 23 <br />1-02.6.GR1 24 <br />Preparation of Proposal 25 <br /> 26 <br />1-02.6.INST3.GR1 27 <br />Section 1-02.6 is supplemented with the following: 28 <br /> 29 <br />1-02.6.OPT3.NEW.GR1 30 <br />(November 20, 2023) 31 <br />The Bidder shall submit with the Bid the following: 32 <br />1) Disadvantaged Business Enterprise Utilization Certification (WSDOT Form 33 <br />272-056) 34 <br />2) DBE Written Confirmation Form (WSDOT Form 422-031) - For each and every 35 <br />DBE firm listed on the Bidder’s completed Disadvantaged Business Enterprise 36 <br />Utilization Certification, the Bidder shall submit written confirmation from that 37 <br />DBE firm that the DBE is in agreement with the DBE participation commitment 38 <br />that the Bidder has made in the Bidder’s completed Disadvantaged Business 39 <br />Enterprise Utilization Certification. 40 <br />3) Good Faith Effort Documentation - Bidder must submit good faith effort 41 <br />documentation with the Disadvantaged Business Enterprise Utilization 42 <br />Certification ONLY In The Event the bidder’s efforts to solicit sufficient DBE 43 <br />participation have been unsuccessful. 44 <br />4) DBE Item Breakdown (WSDOT Form 272-054) The Bidder shall submit a DBE 45 <br />Item Breakdown form defining the scope of work to be performed by each DBE 46 <br />listed on the DBE Utilization Certification. 47
