CITY OF EVERETT <br /> EVERETT Permit Services <br /> WASHINGTON <br /> or installation of hard surfaces (new or replaced), and all work must be performed on existing <br /> infrastructure. New installation of equipment or upgraded equipment (poles, pole-mounted <br /> equipment, antennas, small cell technology, etc.) is not considered a minor activity under this <br /> permit. <br /> b. For Major Activities: as defined in Exhibit A,there are two types of permits. <br /> i. In addition to having an active Annual Blanket Utility Permit, one of the following two <br /> permit types is required to be obtained from the City of Everett prior to commencement of <br /> any work in the right-of-way, based on the proposed scope of work: <br /> • A Traffic Street Use Permit(TSU) is required for any proposed work that includes <br /> the need for traffic control for temporary closures of lanes of travel or sidewalks <br /> and DOES NOT include any pavement restoration, installation of hard surfaces (new <br /> or replaced), or installation of new infrastructure (poles, pole-mounted equipment, <br /> antennas, small cell technology, etc.). An example is aerial work only with <br /> temporary closure of one travel lane and/or the sidewalk. These permits do not <br /> require inspection since there is no permanent restoration or installation of <br /> infrastructure being installed. <br /> • A Franchise Utility Permit (FRU) is required for any proposed work that includes <br /> restoration or installation of hard surfaces (new or replaced), and/or installation of <br /> new infrastructure (poles, pole-mounted equipment, antennas, small cell <br /> technology, etc.). Traffic control for temporary closures of lanes of travel or <br /> sidewalks is to be included in the Franchise Utility permit and does not require a <br /> separate TSU permit. If any of the above applies, a Franchise Utility Permit is <br /> required, pursuant to EMC 13.32. These permits do require inspection since <br /> permanent restoration and/or infrastructure is being installed. <br /> ii. Online Permitting Requirement: Both the Traffic Street Use Permit and Franchise Utility <br /> Permit must be applied for online through the City's eTRAKiT online permitting portal. Once <br /> the permit type is selected, application instructions will appear in a hyperlink next to the <br /> permit type for guidance on how to correctly fill out the application. This must be followed <br /> for the permit application to be considered complete and valid for review and processing. <br /> • Permit Application Contact Information: The contact information must be filled <br /> out correctly to include who the utility company is and who the contractor or <br /> subcontractor will be performing the work. The City of Everett business license is <br /> required to be associated with the contractor performing the work. Only <br /> contractors with a valid City of Everett Business License are allowed to perform <br /> work within Everett City Limits. If prior to job start, the contractor listed on the <br /> permit changes, please contact Everett Permit Services to update the information <br /> and in the inspection request online, add to the notes field who the contractor will <br /> be. Please visit https://Pw.everettwa.gov for all permitting and inspection <br /> scheduling needs. <br /> • Permit Application Upload Requirements: Complete plans, traffic control plans, <br /> and a vicinity map must be uploaded with the online permit application. <br /> • Construction Plans must include the location of all existing and proposed <br /> underground utilities in proximity to the work being performed (if <br /> trenching, directional boring, or setting new infrastructure) and must show <br /> all planned pothole locations. All potential utility conflicts must be <br /> identified with a plan for protection. Note that potholing is required when <br /> crossing City of Everett water lines and the City's Public Works Inspector <br /> must be present during the potholing. <br /> Page 2 of 8 <br />
