City of Everett Lift Station No. 1 Valve Replacement <br /> <br /> Scope of Services 4 <br />4. Copies of subconsultant invoice statements, as applicable, will be provided with monthly <br />invoices. Incidental miscellaneous expenses (travel reimbursement, etc.) do not require <br />submittal of backup statements. <br />Deliverables <br />1. Scope of Services, Schedule (Gantt chart in MS Project or Excel), and Budget (e-mailed <br />Adobe PDF file) for engineering services during construction. <br />2. Monthly status reports and invoices (e-mailed Adobe PDF files). <br />3. Written summary notes (e-mail) describing decisions, direction, action items, or issues <br />associated with scope and budget discussed during monthly project management <br />meetings. <br />Task 200 – Permitting Support <br />Objectives <br />Assist the City in completing the required reviews and obtaining the necessary permits to <br />construct the proposed improvements at the LS-1 site. BHC will utilize Landau Associates as a <br />subconsultant to help with this task. <br />BHC Services <br />1. BHC, with assistance from its subconsultant, will complete the SEPA checklist for the <br />proposed improvements at the LS-1 site. <br />2. BHC and its subconsultant will prepare and submit a pre-application for a Hydraulic <br />Project Approval (HPA) to the Washington State Department of Fish and Wildlife <br />(WDFW) for determining if an HPA is required for the proposed work at LS-1. If <br />necessary, BHC will prepare and submit an HPA application for approval by WDFW. <br />3. BHC and its subconsultant will prepare a Critical Areas Report for the proposed <br />improvements at LS-1. <br />4. BHC will prepare and submit an application for a Utility License Agreement from BNSF <br />for the proposed improvements at LS-1. <br />5. BHC will prepare and submit a Shoreline Permit application for the proposed <br />improvements at LS-1. <br />City of Everett Responsibilities <br />1. Review the SEPA checklist and provide comments to BHC. <br />2. Complete the SEPA process or determine that the proposed improvements are exempt. <br />3. Review the Shoreline Permit application for the proposed improvements at LS-1. <br />4. Review the draft Critical Areas Report and provide comments to BHC for the proposed <br />improvements at LS-1. <br />5. Provide input on the HPA and Utility License Agreement applications, as requested by <br />BHC, for the proposed improvements at LS-1. <br />6. Pay all permit fees.
