Page 10 of 27 <br />o Flexible –Case managers and intervention specialists can flex their schedules when necessary to <br />accommodate special program needs. <br />o Community-Based – Ability to meet clients in the Everett Geographic area and the willingness and <br />ability to ensure transportation for those served to appointments and other suppliers as <br />necessary. <br />o Collaborative – Develop individual care plans and complete referrals for funding, housing, and <br />assessments for substance use disorder (SUD), mental health, medical, dental, and any other <br />needs as requested. <br />o Client Oriented – Provide outreach to individuals by meeting them where they are while assisting <br />them with progress towards being sheltered and addressing needs. For example, street outreach <br />requires the ability to navigate city streets on foot easily and tolerate a variety of weather <br />conditions and terrain. <br />5. Provide all necessary equipment and supplies such as phones, other office needs, data tracking, case <br />management software systems, vehicles, or other transportation needs for case managers. <br />6. Provide adequate resources and information for Case Managers to assist individuals in the City of <br />Everett, including, but not limited to, housing, mental health, physical health, Substance Use Disorder, <br />the Department of Social and Health Services, dental care, community supports, clothing, and food. <br />7. Facilitate partnership among families, the City of Everett, courts, and other treatment providers and <br />systems in which the client is involved. <br />8. Meet with the Community Support Manager or representative once per month to review cases and <br />collaborate regarding case management programming. <br />9. Ensure Case Managers and intervention specialists provide continuity of care, oversight, and access to <br />and coordination of services to meet individualized client goals. <br />10. The Supplier agrees to comply with 2 CFR 200 Uniform Administrative Requirements and agrees to <br />adhere to the accounting principles and procedures required therein, utilize adequate internal <br />controls, and maintain necessary source documentation for all costs incurred. <br />2.7 KEY PERSONNEL <br />The Supplier must provide the following key personnel: <br />1. Case Managers—Two to Three full-time case managers will work alongside the City’s Community <br />Support Team to provide direct case management and navigation services for referred clients. Case <br />Managers are responsible for ensuring appropriate and stable placements and services and promoting <br />healing and recovery for those living in crisis. <br /> <br />2. Intervention Specialist- One full-time intervention specialist will work alongside the City’s Community <br />Support team to provide direct intervention services for those referred. <br />3. Care Management Supervisor – part-time supervisor to ensure contractual agreements are met and <br />provide necessary data and reports. The Care Management Supervisor will also supervise case <br />managers and intervention specialists to ensure they meet expected outcomes. <br />The supplier must furnish the City of Everett with the names of staff to be assigned to the program before <br />service start-up and with any change of staff or upon request thereafter for the term of the contract. <br />2.8 STAFF EXPERIENCE AND CAPABILITIES <br />Staff must be proficient and have sufficient training and abilities to provide the requested services.
