<br />32 <br /> <br />ARTICLE 12 - INSURANCE BENEFITS <br /> <br />Section 1: Eligibility. <br /> <br />a. New hire employees shall receive any City paid insurance benefits the first day of the <br />month following employment (employee starts work in April, insurance starts May 1st). <br />b. Regular part-time employees who work 22 hours or more per week shall be eligible for <br />benefits under Article 12 on a pro-rata basis. <br />c. Day laborers (IV) will be eligible to participate in medical insurance. Returning Day <br />Laborer IV’s shall receive medical insurance on the first day of the following month of <br />rehire <br />d. Circulation Assistant I/Shelvers are exempt from Article 12 unless required by law. <br /> <br />Section 2: Medical Insurance. <br /> <br />The City agrees to offer medical coverage for eligible employees and their legal dependents. Employees <br />and their dependents shall have the option of enrolling in the Kaiser Legacy Core HMO, Kaiser New Core <br />HMO, or one of the City’s self-insured medical plans. Employees that choose the Kaiser HMO Plans or <br />the City’s traditional PPO Plan, shall pay ten percent of the monthly premium for employees and <br />dependents. Employees that choose the City’s Consumer Driven Healthcare Plan (CDHP), shall pay 5% <br />of the monthly premium for employees and dependents. <br /> <br />Employees that enroll in the CDHP will receive a City paid contribution to an HRA/VEBA in the amount <br />of $1,200 for individual for $2,400 for family coverage in the second pay period of the new year. Changes <br />due to a qualifying event will be pro-rated for the remainder of the year. <br /> <br />Annual Whole Health Evaluation: <br />Employees enrolled in the CDHP will receive an HRA/VEBA contribution in the amount of $200 for <br />individual and $200 for spouse or domestic partner upon completion of an Annual Whole Health <br />Evaluation. <br /> <br />Employees enrolled in the traditional PPO Plan will receive an HRA/VEBA contribution in the amount of <br />$100 upon completion of an Annual Whole Health Evaluation. <br />The benefits under the City’s self-insured medical insurance program shall not be reduced without <br />agreement of the parties. <br /> <br />Section 3: Life Insurance. <br /> <br />The City will provide for the employee $1,000 life insurance per $1,000 of annual salary, rounded to the <br />next highest $1,000 at the City's expense for the duration of this agreement. <br /> <br />Section 4: AD&D. <br /> <br />The City will provide for the employee Accidental Death and Dismemberment coverage in an amount <br />equal to that specified in Section 3 hereinabove, at the City's expense for the duration of this agreement. <br />Section 5: Dental. <br /> <br />The City will pay the premium for the dental plan 12, offered through the trustees of the Washington <br />State Council of County and City Employees for the employee and their eligible dependents. <br /> <br />
