Page 10 of 25 <br />D.Diversity – Staff must understand diverse cultures and incorporate the relevant needs of culturally diverse <br />groups, such as people with disabilities, into clinical practice. <br />E. Decision Making – Staff must have the ability to make appropriate decisions to ensure vehicle and client <br />safety. <br />F. Safety and Emergency Procedures – Staff must be thoroughly knowledgeable in the safety and emergency <br />policies and procedures established by the Supplier and the City of Everett. <br />2.9 REPORTING AND TRACKING MEASURES <br />Client tracking and data reporting must be documented in a form acceptable to the City and provided to the <br />City of Everett’s Community Support Coordinator or representative monthly. The following are the minimum <br />key data points that will be measured, tracked, and reported on: <br />1. Number of unique individuals served. <br />2. Transportation locations such as substance use services, mental health services, and medical or social <br />service programs. <br />3. Number of individuals referred to other transportation programs such as Hopelink. <br /> <br />2.10 CITY OF EVERETT RESPONSIBILITIES & PROVIDED SERVICES <br />City of Everett staff will be responsible for and provide the following: <br /> The City of Everett Community Support Manager will oversee all aspects of the contract, including, but not <br />limited to, monitoring and reviewing the supplier’s performance, reports, and data provided under this <br />contract, approving submitted invoices and approving all changes in the contract. <br /> The City of Everett Community Support Manager will support the client referral process. <br /> Review and approve the qualifications and experience of staff providing service under this contract, <br />including new and replacement staff proposed by the Supplier. The City of Everett also reserves the right to <br />require contractor staff to be replaced because of justifiable complaints by program customers, their legal <br />guardians, support systems, or allied agencies. <br />2.11 QUALITY ASSURANCE <br />The Supplier must ensure staff is eligible to provide transportation services for individuals and, at minimum, <br />must provide: <br />1. Education & Experience Verification <br /> The supplier will select transportation specialists for this position based on having experience working <br />with the populations described. <br /> The supplier will select transportation specialists with valid Washington State Driver's licenses who can <br />be insured to drive a vehicle. <br />2. Background Check <br />Before hiring any employee, the Supplier must obtain a nationwide criminal background check. Records of <br />criminal background checks must be kept on file by the Supplier and made available to the City of Everett <br />for review upon request. Proposers shall describe their source and method of obtaining background <br />checks as part of their proposal as part of their answer to Form 4.03, 2G. After work begins, any added or <br />new employees, volunteers, or interns of the Contractor must also comply with the city of Everett’s <br />security background check requirement, as stated above. <br />3. Safety & Emergency <br />The proposer must provide the City of Everett with copies of its agency’s written safety and emergency <br />policies and procedures as part of their proposal response to Form 4.03, 2G. <br />EXHIBIT D
