• <br /> The lead agency for this proposal has determined that it does not have a probable significant <br /> adverse impact on the environment. An Environmental Impact Statement is not required under <br /> RCW 43.21C.030(2)(c). This determination assumes compliance with State law and City <br /> ordinances related to general environmental protection including but not limited to right-of-way <br /> improvement requirements,drainage, etc. This decision was made after review of a completed <br /> environmental checklist and other information on file with the lead agency. This information is <br /> available to the public on request. This Addendum is specifically conditioned on compliance <br /> with the conditions attached hereto which are incorporated by reference as if fully set forth <br /> herein. <br /> This Addendum to MDNS No. 06-014 is issued under 197-11-355. The City as the lead agency <br /> has determined that significant adverse environmental impacts are unlikely; therefore through the <br /> optional DNS process, comments must be submitted by October/1,2011 or fourteen(14)days <br /> after the date shown on the notarized copy of the notice of posting,whichever date is later. <br /> INFORMATION FOR DEVELOPER: <br /> 1. A Public Works Permit is required for this project. Detailed drawings in accordance with <br /> City Design and Construction Standards shall be submitted to the Public Works Department, <br /> showing site parking layout, landscaping,utilities, storm drainage,temporary construction <br /> erosion control, and all required improvements in the public right-of-way. Public Works <br /> Department approval of these drawings is required prior to any permits being issued. All <br /> improvements shall be completed, approved and warranted before the Occupancy Permit is <br /> issued/final approval is granted. <br /> 2. The Applicant and contractor shall minimize impacts to traffic along E. Marine View Drive. <br /> 3. Replace broken curb and sidewalk section as needed where trucks are entering and exiting <br /> the site. <br /> 4. Any grading/fill on this site shall be done so as to not impact the surrounding properties. Fill <br /> should be at least 10 feet away from the City sewer manhole on the south end of the property. . <br /> 5. Fill contractor must have self contained wheel wash system equal to Stanton portable tire <br /> wash,Must filter wash water. During dry weather contractor must keep water truck onsite for <br /> dust control. Additionally may require need for vacuum/sweeper. <br /> 6. Storm water shall be managed in accordance with the City's 2010 Storm water Management <br /> Manual <br /> 7. The Applicant shall notify Everett transit of any potential street or traffic disruptions that <br /> may affect our bus services in the area, <br /> 8. City streets are to be kept clear of dirt and debris at all times during construction. Dust <br /> suppression and street cleaning must occur as directed by the Public Works Inspector. <br /> 9. Lack of comment on the plans submitted with the application does not constitute approval of <br /> these plans. <br /> 3 <br />
