2025 FEDERAL OVERLAY, W MUKILTEO BLVD – GLENWOOD AVE TO DOGWOOD DR <br /> PAGE SP8 OF SP123 SPECIAL PROVISIONS <br />WORK ORDER #3842 April 15, 2025 FEDERAL AID# NHPP-2776(011) <br /> 1 <br />1-02.6(1) Recycled Materials Proposal 2 <br />(January 4, 2016 APWA GSP) 3 <br /> 4 <br />The Bidder shall submit with the Bid, its proposal for incorporating recycled materials into 5 <br />the project, using the form provided in the Contract Provisions. 6 <br /> 7 <br />1-02.7.RTF 8 <br />1-02.7 Bid Deposit 9 <br />(March 8, 2013 APWA GSP) 10 <br /> 11 <br />Supplement this section with the following: 12 <br /> 13 <br />Bid bonds shall contain the following: 14 <br />1. Contracting Agency-assigned number for the project; 15 <br />2. Name of the project; 16 <br />3. The Contracting Agency named as obligee; 17 <br />4. The amount of the bid bond stated either as a dollar figure or as a percentage which 18 <br />represents five percent of the maximum bid amount that could be awarded; 19 <br />5. Signature of the bidder’s officer empowered to sign official statements. The signature 20 <br />of the person authorized to submit the bid should agree with the signature on the 21 <br />bond, and the title of the person must accompany the said signature; 22 <br />6. The signature of the surety’s officer empowered to sign the bond and the power of 23 <br />attorney. 24 <br /> 25 <br />If so stated in the Contract Provisions, bidder must use the bond form included in the 26 <br />Contract Provisions. 27 <br /> 28 <br />If so stated in the Contract Provisions, cash will not be accepted for a bid deposit. 29 <br /> 30 <br />1-02.9.OptionA.RTF 31 <br />1-02.9 Delivery of Proposal 32 <br />(July 8, 2024 APWA GSP, Option A) 33 <br /> 34 <br />Delete this section and replace it with the following: 35 <br /> 36 <br />DBE DOCUMENT SUBMITTAL REQUIREMENTS 37 <br />General 38 <br />Each Proposal shall be submitted in a sealed envelope, with the Project Name and 39 <br />Project Number as stated in the Call for Bids clearly marked on the outside of the 40 <br />envelope, or as otherwise required in the Bid Documents, to ensure proper handling and 41 <br />delivery. 42 <br /> 43 <br />To be considered responsive on a FHWA-funded project, the Bidder may be required to 44 <br />submit the following items, as required by Section 1-02.6: 45 <br /> 46 <br />· DBE Utilization Certification (WSDOT 272-056) 47 <br />· DBE Written Confirmation Document (WSDOT 422-031) from each DBE firm 48 <br />listed on the Bidder’s completed DBE Utilization Certification 49 <br />· Good Faith Effort (GFE) Documentation (if applicable) 50
