Pole and foundation to be replaced <br />with new foundation and pole/ proposed new pole shall replace existing utility light pole and <br />foundation in same location/ fiber and power permits by others. <br />LOCATION: 4030 Colby Ave <br />PERMIT#: PW1908-039 <br />This document serves as the formal correction letter for the above referenced Public Works Permit <br />Application. This permit was applied for on 8/20/2019. An informal correction letter was provided via email <br />for all three small cell sites on day 30 of the review (9/19/2019). Today (9/27/2019) is day 38 of the review. <br />With providing this formal correction letter, the City is tolling the shot clock as of this date. Upon resubmittal <br />with corrections, the shot clock will continue. If you feel that a formal tolling agreement is necessary in <br />addition to this correction letter, please contact Sabrina Fandler (Sfandler@everettwa.eov) and the tolling <br />agreement form will be provided. <br />A review of this project by the Engineering/Permit Services Division indicates that the following revisions shall <br />be made and/or the following conditions must be met: <br />1. Please resubmit the following items to the Permit Services Counter with your resubmittal once all <br />requested revisions have been made: <br />a. Transmittal Sheet: (1) available at the Permit Counter <br />b. Plans: (4) full size sets of revised plans <br />The Permit Counter is open Monday through Friday from 7:30am-12:00pm and from 1:00pm- <br />4:00pm (closed between 12:00pm-1:00pm for lunch). <br />2. The Site license addendums are incorrectly cross referencing each other. Please revise to ensure <br />that the site license addendum number and address are correctly correlated.
