Pole and foundation to be replaced with new foundation and pole/ proposed new pole shall replace existing utility light pole and foundation in same location/ fiber and power permits by others. LOCATION: 4030 Colby Ave PERMIT #: PW1908-039 A review of this project by the Engineering/Permit Services Division indicates that the following revisions should be made and/or the following conditions must be met: Please see the compiled list of comments below to revise your plans accordingly, prior to resubmitting to Permit Services. Please respond to comments in the table's response column provided. If you have any questions regarding the comments/redlines or need anything further, please directly contact the listed reviewers. Electronic Resubmittal: Please send an email with attachments or a downloadable link to SGood@everettwa.eov, cc: ERichards@everettwa.eov and DByrd@everettwa.gov to resubmit the following permit application documents: i. Completed Transmittal sheet (PDF) for your use upon resubmittal. ii. Plans: Revised complete set of all construction plans (civils, illumination plans, pole specs, etc.) iii. Site License Addendum (SLA)- marked up with your proposed revisions and filled out to be site specific with the correct address of 4030 Colby Ave. We anticipate some language changes are necessary to describe the power source. iv. Cost Estimate for Permit Fee Calculation Form: (1) copy of cost estimate form filled out for the City to use to calculate the Public Works Permit Fee. Document attached to Correction Letter email for your use.
