2025 FEDERAL OVERLAY, BROADWAY – CALIFORNIA ST TO 18TH ST SPECIAL PROVISIONS <br />FEDERAL AID# STBGUL-2715(012); COE PW# 3841 SP-7 May 6, 2025 <br /> <br />A bid by a corporation shall be executed in the corporate name, by the president or a 1 <br />vice president (or other corporate officer accompanied by evidence of authority to sign). 2 <br />A bid by a partnership shall be executed in the partnership name and signed by a 3 <br />partner. A copy of the partnership agreement shall be submitted with the Bid Form if any 4 <br />DBE requirements are to be satisfied through such an agreement. 5 <br />A bid by a joint venture shall be executed in the joint venture name and signed by a 6 <br />member of the joint venture. A copy of the joint venture agreement shall be submitted 7 <br />with the Bid Form if any DBE requirements are to be satisfied through such an 8 <br />agreement. 9 <br /> 10 <br />The Bidder shall submit the following supplemental documents with the Bid in accordance 11 <br />with 1-02.9: 12 <br />1) Disadvantaged Business Enterprise Utilization Certification (WSDOT Form 272-056) 13 <br />2) DBE Written Confirmation Form (WSDOT Form 422-031) - For each and every DBE 14 <br />firm listed on the Bidder’s completed Disadvantaged Business Enterprise Utilization 15 <br />Certification, the Bidder shall submit written confirmation from that DBE firm that the 16 <br />DBE is in agreement with the DBE participation commitment that the Bidder has 17 <br />made in the Bidder’s completed Disadvantaged Business Enterprise Utilization 18 <br />Certification. 19 <br />3) Good Faith Effort Documentation - Bidder must submit good faith effort 20 <br />documentation with the Disadvantaged Business Enterprise Utilization Certification 21 <br />ONLY In the Event the bidder’s efforts to solicit sufficient DBE participation have been 22 <br />unsuccessful. 23 <br />4) DBE Item Breakdown (WSDOT Form 272-054) The Bidder shall submit a DBE Item 24 <br />Breakdown form defining the scope of work to be performed by each DBE listed on 25 <br />the DBE Utilization Certification. 26 <br />Directions for delivery of the Disadvantaged Business Enterprise, Written Confirmation 27 <br />Documents, and Disadvantaged Business Enterprise Good Faith Effort documentation are 28 <br />included in Sections 1-02.9 and 1-02.10. 29 <br /> 30 <br />1-02.6.OptionC.RTF 31 <br />Subcontractor’s List 32 <br />(November 25, 2024 APWA GSP 1-02.6, Option C) 33 <br />The fourth paragraph of Section 1-02.6 is revised to read: 34 <br /> 35 <br />The Bidder shall submit with the Bid the completed Subcontractor List included in the 36 <br />Contracting Agency Proposal Package. If a Subcontractor List Form is not included in 37 <br />the package, use DOT Form 271-015LP. The Form shall contain the following: 38 <br /> 39 <br />1. Subcontractors who will perform the work of structural steel installation, rebar 40 <br />installation, heating, ventilation, air conditioning, and plumbing as described in 41 <br />RCW 18.106 and electrical as described in RCW 19.28, 42 <br />2. The Work those subcontractors will perform on the Contract as described in 43 <br />RCW 39.30.060; and 44 <br />3. No more than one subcontractor for each category of work identified, except, 45 <br />when subcontractors vary with Bid alternates, in which case the Bidder shall 46 <br />identify which subcontractor will be used for which alternate. 47 <br /> 48
