Contracting Agency-assigned number for the project; 17 <br />2. Name of the project; 18 <br />3. The Contracting Agency named as obligee; 19 <br />4. The amount of the bid bond stated either as a dollar figure or as a percentage which 20 <br />represents five percent of the maximum bid amount that could be awarded; 21 <br />5. Signature of the bidder’s officer empowered to sign official statements. The signature 22 <br />of the person authorized to submit the bid should agree with the signature on the 23 <br />bond, and the title of the person must accompany the said signature; 24 <br />6. The signature of the surety’s officer empowered to sign the bond and the power of 25 <br />attorney. 26 <br /> 27 <br />If so stated in the Contract Provisions, bidder must use the bond form included in the 28 <br />Contract Provisions. 29 <br /> 30 <br />If so stated in the Contract Provisions, cash will not be accepted for a bid deposit. 31 <br /> 32 <br />1-02.9.OptionA.RTF 33 <br />1-02.9 Delivery of Proposal 34 <br />(April 22, 2025 APWA GSP, Option A) 35 <br />Delete this section and replace it with the following: 36 <br />GENERAL 37 <br />Each Proposal shall be submitted in a sealed envelope, with the Project Name and 38 <br />Project Number as stated in the Call for Bids clearly marked on the outside of the 39 <br />envelope, or as otherwise required in the Bid Documents, to ensure proper handling and 40 <br />delivery. 41 <br />To be considered responsive on a FHWA-funded project, the Bidder may be required to 42 <br />submit the following items, as required by Section 1-02.6: 43 <br />· DBE Utilization Certification 44 <br />· DBE Written Confirmation Document (from each DBE firm listed on the Bidder’s 45 <br />completed DBE Utilization Certification 46 <br />· Good Faith Effort (GFE) Documentation (if applicable) 47
