CITY OF EVERETT BID SET <br />EVERETT STATION HVAC AND CONTROLS IMPROVEMENTS <br />PROJECT NO. 24-3201 <br /> <br />01 35 29 -2 HEALTH AND SAFETY <br /> <br /> If work requires the use of Fall Restraint or Fall Arrest, a Fall Protection work plan meeting the <br />requirements set forth in WAC 296-155-24505 shall be included. <br /> If applicable, the Plan shall include the name and qualifications of any electrical safety observer to <br />be provided by the Contractor. <br /> Steps taken to provide Protection of the Public and City Employees. This includes protection from <br />falling objects. Indicated type of overhead safe guards and protection to be installed and safeguards <br />from equipment damage due to debris (including nails) on the ground. <br /> A description of tasks to be undertaken, and equipment mobilized for this project. <br /> A list of all known and expected safety or health hazards, problems, and proposed control <br />mechanisms. <br /> Material Safety Data Sheets (MSDS) of and procedures for using, disposing of, or storing for all <br />chemicals, products, or materials regulated by WAC 296-62 to be used by the Contractor. <br /> A list of personal protective equipment, monitoring devices, and hazard-specific plans or permits as <br />appropriate and required by State and Federal regulations. <br /> A description of emergency response measures, equipment available for emergency response to <br />address accidents and releases of materials, including, but not limited to, first aid, eye <br />wash/showers, and fire extinguishing equipment, and location of this equipment at the jobsite. <br /> Emergency phone numbers contacts, and location of the nearest medical facility. <br /> A monitoring and inspection plan and record keeping measures to ensure that equipment and work <br />practices comply with this Plan. <br /> Personnel names, training and notification procedures as appropriate to ensure that all jobsite <br />personnel are familiar with the Plan elements. Include copies of training certificates. <br /> Procedures for safe storage and handling of flammable liquids, in accordance with WAC 296-24- <br />330. <br /> If applicable the Contractor shall include procedures for safe storage and handling of compressed <br />gasses in accordance with WAC 296-24-295, Compressed Gas General Requirement. <br /> Other issues which the Contractor determines are appropriate and necessary to protect worker <br />safety and health. <br /> This plan shall be posted and marked in a location on site that will be noticed upon entry of work <br />site. A fall protection plan must also be established and shall be posted next to the Health and <br />Safety Plan (Accident Prevention Program). These postings are required to be protected from <br />inclement weather and must be legible during the extent of this contract. <br />1.06 HEALTH AND SAFETY REPRESENTATIVE <br /> The Contractor shall designate a Health and Safety Representative who will oversee all work taken place on <br />the job site including work performed by subcontractors and suppliers. The Health and Safety Rep should be <br />capable of identifying hazards within the job site, take immediate actions to correct hazard and if needed, <br />stop work until hazards have been corrected. <br /> The Contractor shall authorize the Health and Safety Representative to resolve safety-related issues raised by <br />the Owner.
