<br />PGSF WMVD Storm and Combined Sewer - Construction Package 2 Removal and Disposal of Contaminated Soils <br />Project No. 180162 02 61 00 - 3 <br /> <br />c. Methods and procedures of excavation and equipment to be used. Include <br />methods to prevent the spread of Contaminated Substances. <br />d. Staging and storage methods, procedures, and locations for liquid and solid <br />contaminated material. Include methods to prevent noncontaminated soil <br />excavated during sloping sidewalls from mixing with contaminated soil. <br />e. Backfill material sources including chemical analytical data or plan to obtain <br />chemical analytical data prior to use of material for backfill. <br />f. Methods and procedures for the transportation, disposal, and off-site treatment <br />of contaminated materials, in compliance with applicable federal, state, and local <br />laws and regulations, including the identification of disposal and treatment <br />facilities, and the use of certified, licensed transporters. <br />g. Equipment decontamination procedures to prevent cross contamination. <br />h. Sampling and analysis plans and responsibilities, including the characterization <br />of materials for disposal, and releases of hazardous and contaminated <br />substances. <br />i. Relevant additions to project-wide Spill Prevention, Control, Contingency, and <br />Countermeasures (SPCCC) plan <br />j. Methods for water management including groundwater, stormwater runoff, and <br />surface water. <br />k. The name, address, and state and federal identification number (if applicable) of <br />all off-site soil treatment or disposal facilities <br />l. Procedures for documenting and reporting encounters with and/or releases of <br />hazardous or contaminated substances <br />m. Coordinate with requirements of Section 01 35 29 Health and Safety. <br />n. Coordinate with requirements of the Erosion and Settlement Control 31 25 00. <br />o. Coordinate with requirements of Section 31 23 19, Dewatering. <br />p. Obtain all required permits and notifications for removal, excavation, dewatering, <br />storage, transportation, and disposal of contaminated material. <br />C. EXCAVATION DOCUMENTATION. The contractor shall document intrusive activities, all <br />environmental test data, post intrusive activity environmental conditions, and identify any <br />changes proposed to any applicable restrictive covenant as required by the SGMP. <br />D. Closeout Documents at the completion of the task, including: <br />1. Chains-of-custody, Analytical laboratory test results <br />2. Waste manifests, truck weight tickets <br />3. Disposal facility certificates of disposal <br />1.05 QUALITY ASSURANCE <br />A. Qualifications <br />1. Site supervisor: <br />a. Trained and experienced in hazardous and contaminated material handling <br />b. Completed OSHA training requirements for working with hazardous substances <br />including the 8-hour supervisory course <br />c. Minimum of three (3) years of experience in managing hazardous materials <br />projects
