Furnish the original return copy of the hazardous waste manifest, signed by the <br />owner or operator of a facility legally permitted to treat or dispose of those <br />materials furnished to the Engineer not later than five (5) days following the <br />delivery of those materials to the facility. <br />c. Furnish a statement of agreement from the proposed treatment, storage or <br />disposal facility and certified transporters to accept hazardous or special wastes <br />in the CMH Plan. <br />d. Provide copies of all trucking tickets from the transport of contaminated <br />materials to the Engineer on a weekly basis. <br />9. Contractor shall remove from the site and dispose all equipment and materials from <br />the site at the conclusion of the job, including visqueen, soil liners, covers, straw <br />bales, personal protective equipment and other materials at Contractor's expense, in <br />accordance with all applicable regulations. <br />3.05 FIELD QUALITY CONTROL <br />A. Sampling, Screening, and Analysis <br />1. Have the independent testing firm employed by the Contractor perform required <br />sampling and chemical analyses relating to generation, use, release, and disposal of <br />contaminated substances in the course of operations, in accordance with the <br />Contaminated Material Handling Plan. <br />2. Have the independent testing firm employed by the Contractor perform required <br />sampling and chemical analyses relating to existing potentially contaminated <br />substances unless otherwise provided herein or in the Contract Documents. Include <br />characterization sampling and the sampling necessary to determine disposal <br />methods in this sampling. Do not dispose of material until directed to do so by the <br />Engineer. <br />3. Submit results of all screening testing to the Engineer. <br />4. The Engineer will inspect the removal of existing contaminated material from each <br />site. Notify the Engineer at least 48 hours prior to when confirmation sampling and <br />analysis will be required. Mark locations of samples in the field and document on the <br />surveys and the as-built drawings. <br />3.06 DECOMMISSIONING OF MONITORING WELLS <br />A. Approximately 8 monitoring well exist on the site. The names and locations of 7 are <br />presented in the Geotechnical report in Volume 4 Attachment B for HWA-7P-24, HWA- <br />7VW-25, HWA-19P-24, HWA-20P-24, HWA-14P-25, HWA-3P-24, HWA-3VW-25. <br />Additionally, one City of Everett Well (ID BIZ-212) located near HWA-7P-24 shall be <br />decommissioned. <br />B. Contractor shall decommission selected monitoring wells, as shown in the geotechnical <br />report, or as directed by Engineer, after all dewatering work in that area is completed. <br />C. Contractor shall decommission the wells in accordance with Chapter 173-160 WAC <br />Minimum Standards for Construction and Maintenance of Wells. <br />D. Work shall be performed by a Drilling Contractor licensed under Chapter 173-162 WAC, <br />Regulation and Licensing of Well Contractors and Operators.
