2026 Pavement Maintenance Overlay Special Provisions <br />PW# 3860 SP - 5 March 5, 2026 <br />1 of addenda; the bidders name, address, telephone number, and signature. Bids shall be <br />2 in legible figures (not words) written in ink or typed and expressed in U.S. dollars. The <br />3 required certifications are included as part of the Proposal Form. <br />4 <br />5 The Contracting Agency reserves the right to arrange the proposal forms with alternates <br />6 and additives, if such be to the advantage of the Contracting Agency. The bidder shall bid <br />7 on all alternates and additives set forth in the Proposal Form unless otherwise specified. <br />8 <br />9 1-02.6.GR1 <br />10 Preparation of Proposal <br />11 <br />12 1-02.6.INST3.GR1 <br />13 Section 1-02.6 is supplemented with the following: <br />14 <br />15 1-02.6.OptionA.RTF <br />16 1-02.6 Preparation of Proposal <br />17 (February 17, 2026 APWA GSP, Option A) <br />18 <br />19 Revise the fourth paragraph to read: <br />20 <br />21 Subcontractors List <br />22 The Bidder shall submit with the Bid the completed Subcontractor List included in the <br />23 Contracting Agency Proposal Package. If a Subcontractor List Form is not included in <br />24 the package, use DOT Form 271-015. The Form shall contain the following: <br />25 <br />26 1. Subcontractors who will perform the Work of structural steel installation, rebar <br />27 installation, heating, ventilation, air conditioning, and plumbing as described in <br />28 RCW 18.106 and electrical as described in RCW 19.28, <br />29 <br />30 2. The Work those subcontractors will perform on the Contract and the proof of <br />31 license when required as described in RCW 39.04.350(1), and <br />32 <br />33 3. No more than one subcontractor for each category of Work identified, except, <br />34 when subcontractors vary with Bid alternates, in which case the Bidder shall <br />35 identify which subcontractor will be used for which alternate. <br />36 <br />37 1-02.6.OptionC.RTF <br />38 1-02.6 Preparation of Proposal <br />39 (February 17, 2026 APWA GSP, Option C) <br />40 <br />41 The first sentence of the second paragraph is revised to read as follows: <br />42 <br />43 All prices shall be in legible figures (not words) written in ink or typed, and expressed in <br />44 U.S. dollars. <br />45 <br />46 Supplement the second paragraph with the following: <br />47 <br />48 4. If a minimum bid amount has been established for any item, the unit or lump sum <br />49 price must equal or exceed the minimum amount stated. <br />50 <br />51 Delete the last two paragraphs, and replace them with the following: <br />52
