5. Names and right-of-way widths of all streets within the subdivision or short <br /> subdivision and immediately adjacent to the subdivision or short subdivision. Street <br /> names shall be consistent with the names of existing adjacent streets; <br /> 6. Number of each lot and each block; <br /> 7. Reference to private restrictive covenants not required by the city either to be filed as <br /> a separate document or on the face of the plat or short plat map; <br /> 8. Zoning setback lines and building sites when required by city; <br /> 9. Location, dimensions and purpose of any easements, noting if the easements are <br /> private or public; <br /> 10. Location and description of monuments and all lot corners set and found; <br /> 11. Datum elevations and primary control points approved by the public works <br /> department. Descriptions and ties to all control points will be shown with dimensions, <br /> angles, and bearings; <br /> 12. Existing structures and all setbacks; <br /> 13. A dedicatory statement acknowledging public and private dedications and grants; <br /> 14. Environmentally sensitive area tract with buffers, greenbelts, and open space tracts, <br /> recreation tracts or trails, and all easements; <br /> 15. Restrictive covenants or plat/short plat conditions required to be noted on the final <br /> plat or short plat map by the city; <br /> 16. Storm water drainage facilities (detention and treatment) tract and landscaped area; <br /> 17. Lot area of each lot and calculation if using ESA or lot area averaging; and <br /> 18. Addresses for each lot. <br /> B. Accompanying the final plat or short plat shall be the following: <br /> 1. A certificate giving a full and correct description of the lands divided as they appear <br /> on the plat or short plat, including a statement that the subdivision or short subdivision <br /> has been made with the free consent and in accordance with the desires of the owners. If <br /> the plat or short plat is subject to a dedication, the certificate or a separate written <br /> instrument shall also contain the dedication of all streets and other areas to the public, an <br /> individual or individuals, religious society or societies or to any corporation, public or <br /> private as shown on the plat or short plat and a waiver of all claims for damages against <br /> any governmental authority which may be occasioned to the adjacent land by the <br /> established construction, drainage and maintenance of the road. The certificate or <br /> instrument of dedication shall be signed and acknowledged before a notary public by all <br /> parties having any ownership interest in the land subdivided and recorded as part of the <br /> final plat or short plat map; <br /> 2. A certification by a licensed surveyor in the state of Washington that the plat or short <br /> plat map and survey is accurate and conforms to the provisions of this title and state law; <br /> 3. Certification by the director that the plat or short plat map conforms to all conditions <br /> of preliminary approval; <br /> 4. Certification by the city engineer that the plat or short plat map conforms to survey <br /> requirements of this title, that the layout of streets, alleys and rights-of-way, design of <br /> bridges, sewage, water and drainage systems, and all other public improvements have <br /> been installed to city standards; <br /> 5. Certification by city council that all plat conditions have been met; <br /> 11 <br />
