1 c. Description of partial items (if any) to be sublet to each <br /> 2 successful DBE firm specifying the distinct elements of Work <br /> 3 under each item to be performed by the DBE and including the <br /> 4 dollar value of the DBE portion. <br /> 5 <br /> 6 Total amounts shown for each DBE shall not be less than the <br /> 7 amount shown on the Disadvantaged Business Enterprise <br /> 8 Utilization Certification. A breakdown that does not conform to <br /> 9 the Disadvantaged Business Enterprise Utilization Certification <br /> 10 or that demonstrates a lesser amount of DBE participation than <br /> 11 that included in the Disadvantaged Business Enterprise <br /> 12 Utilization Certification will be returned for correction. <br /> 13 <br /> 14 2. A list of all firms who submitted a Bid or quote in an attempt to <br /> 15 participate in this project whether they were successful or not. <br /> 16 Include the business name and a mailing address. <br /> 17 <br /> 18 Note: The firms identified by the Prime Contractor may be contacted by the <br /> 19 Contracting Agency to solicit general information as follows: age of <br /> 20 the firm and average of its gross annual receipts over the past three- <br /> 21 years. <br /> 22 <br /> 23 Procedures after Execution <br /> 24 Crediting DBE Participation toward Meeting the Goal <br /> 25 Reporting <br /> 26 All DBE work whether COA or race neutral participation is reported. <br /> 27 The Prime Contractor shall submit a Quarterly Report of Amounts <br /> 28 Credited as DBE Participation form (422-102 EF) on a quarterly basis <br /> 29 for any calendar quarter in which DBE has accomplished Work or <br /> 30 upon completion of the project, as appropriate. The dollars are to be <br /> 31 reported as specified herein. <br /> 32 <br /> 33 In the event that the payments to a DBE have been made by an <br /> 34 entity other than the Prime Contractor, as in the case of a lower-tier <br /> 35 Subcontractor or supplier, then the Prime Contractor shall obtain the <br /> 36 quarterly report, including the signed affidavit, from the paying entity <br /> 37 and submit the report to the Contracting Agency. <br /> 38 <br /> 39 Changes in DBE COA participation <br /> 40 Owner initiated Change Orders <br /> 41 The Prime Contractor shall demonstrate a GFE to substitute COA DBE <br /> 42 participation when the Contracting Agency deletes Work items by change <br /> 43 order that impact a COA DBE's Work. <br /> 44 <br /> 45 When the Contract allows alternate Work methods which serve to delete <br /> 46 or create under-runs in COA DBE Work then the Prime Contractor must <br /> 47 provide documentation of negotiating the change with the DBE that was to <br /> 48 perform the reduced Work and demonstrate a GFE to substitute other <br /> 49 DBE COA participation. <br /> 50 <br /> HAWTHORNE ELEMENTARY <br /> SAFE ROUTES TO SCHOOL <br /> FED AID#SRTS-0420(021);COE#3540 SP-46 March 16, 2016 <br />
