I 1 1-02.4 Examination of Plans, Specifications, and Site of Work I 2 3 1-02.4(1) General I 4 Section 1-02.4(1) is amended by adding the following: 5 Before submitting its proposal, the bidder shall visit the site and familiarize itself with the 6 conditions under which its project is to be performed and correlate its observations with 7 the requirements of all contract plans, specifications, and conditions. I 8 9 1-02.5 Proposal Forms I 10 The first paragraph of section 1-02.5 is amended by deletion of the first paragraph, as follows: 11 A -- - e'er.- , - -- -- -5 A - - - - --- - - -• - .- 12 project on which the bidder is eligible to bid. I 13 14 Section 1-02.5 is further amended by adding the following: 15 The bidder shall submit its proposal on the forms found in the proposal specifications. 111 16 All blank spaces in the proposal form shall be properly filled in. In addition to writing in 17 the bid amount in numbers, be sure and write out the bid in words in the space provided 18 in the Proposal. Where conflict occurs, the written or typed words shall prevail. 19 Qualification of a bid will result in automatic rejection of the bid. I20 The Contracting Agency reserves the right to arrange the proposal forms with alternates 21 and additives, if such are to the advantage of the Contracting Agency. The bidder shall I 22 bid on all alternates and additives set forth in the proposal forms unless otherwise 23 specified. 24 Any correction to a bid made by inter-lineation, alteration, or erasure, shall be initialed by 25 the signer of the bid. The bidder shall make no stipulation on the Bid Form, nor qualify 26 the bid in any manner. 27 A bid by a corporation shall be executed in the corporate name, by the president or a I 28 vice president (or other corporate officer accompanied by evidence of authority to sign). 29 A bid by a partnership shall be executed in the partnership name, and signed by a 30 partner. A copy of the partnership agreement shall be submitted with the Bid Form if any I 31 D/MIWBE requirements are to be satisfied through such an agreement. 32 A bid by a joint venture shall be executed in the joint venture name and signed by a 33 member of the joint venture. A copy of the joint venture agreement shall be submitted 34 with the Bid Form if any D/W/MBE requirements are to be satisfied through such an 35 agreement. 36 I 37 1-02.6 Preparation of Proposal 38 Supplement the second paragraph with the following: 39 4. If a minimum bid amount has been established for any item, the unit price must I 40 equal or exceed the minimum amount stated. 41 42 The first subsection of the fifth paragraph of 1-02.6 is modified to read as follows (addition is 43 underlined): 44 "The bidder shall submit with the bid a list of: 45 1.
