5 <br /> Project: <br /> Change Order no. <br /> Change Order Effective Date: <br /> Item#30—Asphalt Concrete for Preleveling,Class B. The estimated tons of asphalt concrete for <br /> preleveling,class B were exceeded to fill low spots in the grade of Evergreen Way. A significant low spot <br /> in the overall grade along Evergreen Way existed in the vicinity of 42nd to 43`d streets. Some grinding of <br /> the high spots in the existing asphalt surface along portions of Evergreen Way was done,in conjunction <br /> with lowering the new curb and gutter grades,to lesson the need for additional tons of asphalt concrete for <br /> preleveling,class B. <br /> Item#33—Concrete Inlet. The estimated number of concrete inlets were exceeded in a large part do to <br /> the fact that the existing old style inlets would fall apart when worked around. In addition the plans called <br /> for inverts of new pipes into the existing catch basins with grades that were below the bottom catch <br /> elevation of the existing old style catch basins thus requiring the removal of the existing CB and installation <br /> of a new deeper concrete inlet to accommodate the flow elevations. <br /> Item#34—Adjust Manhole to Grade. All manholes that were affected as part of this project were <br /> adjusted to grade as necessary. <br /> Item#37—Adjust Meter Box to Grade. All meter boxes that were affected as part of this project were <br /> adjusted to grade as necessary. <br /> Item#39—Structural Excavation CL A. Those areas that required structural excavation class A to build <br /> the soil nail wall,rock walls and CMU walls along the project were excavated. The City's original cross <br /> sections did not include quantities for structural excavation in several areas where structural excavation <br /> occurred. <br /> Item#40—Concrete Class B. This bid item is not used as a result of the redesigned structural soil nail <br /> wall. <br /> Item#41—Steel Reinforcing Bar. This bid item is not used as a result of the redesigned structural soil <br /> nail wall. <br /> Item#42—Modular Block Retaining Wall. Additional modular block retaining walls were installed in <br /> several areas along the project where the plans were silent on how to hold back cut or fill sections at the <br /> Right-of-Way line. <br /> Item#43—Gravel Backfill for Walls. The majority of the tons allowed for in the bid proposal for gravel <br /> backfill for walls are being deleted as a result of the redesigned structural soil nail wall design. The <br /> balance will be used behind the last 50 feet or so of the north end of the soil nail wall. <br /> Item#44—4'Chain Link Fence Type 6,Black Vinyl. Additional linear feet of 4'black vinyl chain link <br /> fence type 6,were installed in areas where the plans were silent. <br /> Item#45—Resetting Existing Hydrants. Plans called for three locations where existing fire hydrants <br /> require relocation. The bid proposal only allowed for one each. <br /> Item#46—Cement Concrete Curb&Gutter,Type A-1. The City added additional linear feet of cement <br /> concrete curb&gutter,type A-I to the contract work. <br /> Item#49—Existing Private Improvement Restoration. This bid item allowed$20,000.00 to restore the <br /> existing improvements at or near the right-of-way or private property line along the project. $30,000.00 <br /> additional dollars are being added to the cost of this bid item. <br /> _ 6 <br /> Exhibit A <br /> Page 7 of 11 <br />
