15 <br /> Exhibit A <br /> STATEMENT OF WORK <br /> 1. General. The Grantee shall increase participation in National Fire Incident Reporting <br /> System (NFIRS)version 5.0 in accordance with Exhibit B, Grant Application, which is <br /> attached hereto and incorporated herein. <br /> 2. Project Budget. The project budget is the line item budget identified in Exhibit B, with <br /> the following exceptions (if any): <br /> The Grantee's budget is revised to the following: <br /> Computer hardware/software $8,688 <br /> Total $8,688 <br /> The Grantee may shift an amount not to exceed 10% of the Maximum Contract Amount <br /> for each line item with the following exceptions: <br /> • The Maximum Contract Amount identified on Page 1 of this Agreement cannot be <br /> exceeded. <br /> • Funds may not be shifted into a previously unused line item. <br /> Any fund shift in excess of 10% of the Maximum Contract Amount or as identified by the <br /> exceptions above require an amendment to this Agreement in accordance with Section <br /> 5, Agreement Alterations and Amendments. <br /> Any goods or services for which the Grantee seeks reimbursement through this <br /> Agreement must be received no later than May 31, 2004. <br /> 3. Travel Reimbursement. If Travel costs are included in the line item budget contained in <br /> Exhibit B, WSP shall reimburse the Grantee for those costs at State of Washington <br /> approved rates. <br /> 4. Reporting Requirements. <br /> a. NFIRS 5.0 Data Submission. The Grantee shall forward NFIRS data to the WSP <br /> Project Manager at the end of each month. The data may be submitted in electronic <br /> format on 3 '/2 in. floppy disks, Zip disks or compact disks; or via e-mail. <br /> b. Financial Reporting. The Grantee shall submit a final expenditure report and all <br /> remaining requests for reimbursment to the WSP Project Manager identified on Page <br /> 1 of this Agreement by June 1, 2004 or upon early termination of the Agreement. <br /> c. Project Reporting. The Grantee shall submit a project report updating the status of <br /> NFIRS 5.0 progress on the following dates: <br /> • October 1, 2003 <br /> • January 2, 2004 <br /> • April 1, 2004 <br /> 1 3 8 <br /> NFIRS Grant Agreement Page 6 <br /> 8/29/01 <br />
