Determine that a violation of Policy or Procedure has actually <br /> occurred. <br /> 2.) Be responsible for a thorough investigation of the incident, <br /> contacting all persons who have knowledge of or who <br /> participated in the incident, documenting each view. <br /> C. Once the evidence is gathered, a determination as to violation will <br /> occur. <br /> 1.) Actual violations that are substantial in nature can be subject <br /> do disciplinary measures. These measures will be approved <br /> by the Council and carried out. Written, signed, and dated <br /> notifications will be provided to the person(s) involved_ <br /> 2.) Sanctions may include the following; <br /> A.) First violation of any rule-verbal reprimand with <br /> explanation of the appropriate action. <br /> B.) Second violation of any rule- written reprimand with <br /> explanation of the violation and appropriate action. <br /> C.) Third violation ofanyrule within 6 months and review <br /> and approval of two thirds of the Council may result in <br /> 28 <br />
