Project: <br /> Change Order no. <br /> 11 Change Order Effective Date: <br /> Item#10—Sawing Pavement. Original estimated linear feet of sawing pavement were exceeded to <br /> complete work on the project. <br /> Item#12—Remove Existing Pavement,Type 1,Class B. The original estimated quantities of remove <br /> existing pavement,type I class B were not used during construction of the project. <br /> Item#15—Remove cement concrete sidewalk. The original estimated quantities of remove cement <br /> concrete sidewalk were not used during construction of the project. <br /> Item#16—Roadway Excavation Including Haul. The original estimated cubic yards of roadway <br /> excavation including haul were significantly exceeded do to the existence of a hog fuel type material along <br /> the Snohomish County PUD right-of-way just north of Alta Drive,which required removal to construct the <br /> pathway. The decision was made to remove this hog fuel material instead of attempting to bridge over it. <br /> This hog fuel would eventually decompose and settle under the weight of Snohomish County PUD crew <br /> trucks that travel this pathway to service their utility poles in the PUD right-of-way. The City of Everett <br /> and KLB Construction renegotiated the unit price of roadway excavation including haul on those cubic <br /> yards above 125%of the original bid quantity. This is new bid item#81. <br /> Item#17—Remove Catch Basin. The removal of one additional catch basin during construction on this <br /> project was required to complete storm drainage work. <br /> Item#18—Gravel Borrow. The original estimated quantities of gravel borrow were not used during <br /> construction on this project. Much of the existing material along Commercial Avenue was an existing <br /> granular type material. <br /> Item#19—Catch Basin,Type A. Additional catch basins were added along Commercial Avenue and <br /> Alta Drive to prevent water from ponding in the walkway. <br /> Item#20—Adjust Catch Basin to Grade. An additional adjustment of catch basin to grade was required <br /> during construction on the project. <br /> Item#21—Furnish &Install Solid Lid for Catch Basin. The original estimate of furnish&install solid <br /> lid for catch basins were exceeded by two each during work on the project. <br /> Item#22-Storm Sewer Pipe,8-Inch Diameter. The plans indicate installation of 204 linear feet of 8- <br /> inch diameter storm sewer pipe. The original bid proposal aloud for 67 linear feet of 8-inch diameter storm <br /> sewer pipe. Additional linear feet of 8-inch diameter storm sewer pipe were added by the City to convey <br /> collected storm sewer water into existing system. The City of Everett and KLB Construction renegotiated <br /> the unit price for 8-inch diameter storm sewer pipe for those quantities above 125%of the original bid <br /> proposal amount. This is new bid item#78. <br /> Item#23—Storm Sewer Pipe,12-Inch Diameter. The plans indicate installation of 654 linear feet of 12- <br /> inch diameter storm sewer pipe. The original bid proposal aloud for 19 linear feet of 12-inch diameter <br /> storm sewer pipe. The City of Everett and KLB Construction renegotiated the unit price for 12-inch <br /> diameter storm sewer pipe for those quantities above 125%of the original bid proposal amount. This is <br /> new bid item#79. <br /> Item#24—Ductile Iron Storm Sewer Pipe,8-inch Diameter. An additional 35 linear feet of 8-inch <br /> diameter ductile iron storm sewer were added by the City of Everett during the project to convey collected <br /> storm sewer water into the existing system. <br /> Item#25—Connect to Drainage Structure. Three additional connections to existing drainage structures <br /> were added at the direction of the City of Everett to complete storm drainage work on the contract. <br /> Item#26—Adjust Manhole to Grade. The City of Everett deleted one each adjustment of manhole to <br /> grade from the contract. <br /> Item#28—Adjust Valve Box to Grade. The City of Everett deleted five each adjustments of valve boxes <br /> to grade from the contract. <br /> Item#29—Pavement Patch,Asphalt Concrete,Class B.
