Project: <br /> Change Order no. <br /> Change Order Effective Date: <br /> Item#30—Crushed Surfacing Top Course. Additional tons of crushed surfacing top course were added <br /> by the City of Everett to the contract to fill areas along the Snohomish County PUD right-of-way where <br /> existing hog fuel material was removed. Additionally the City of Everett made adjustments to the roadway <br /> section along Commercial Avenue that resulted in the use of more crushed surfacing top course but less <br /> asphalt concrete pavement class B. The City of Everett and KLB Construction renegotiated the unit price <br /> for crushed surfacing top course for those quantities above 125%of the original bid proposal amount. This <br /> is new bid item#82. <br /> Item#31—Asphalt Treated Base. The 24.40 tons of asphalt treated base that were not used on the project <br /> are being deducted from the contract. <br /> Item#32—Asphalt Concrete Pavement,Class B. 647.29 ton of asphalt concrete pavement,class B are <br /> being deducted from the contract amount as a result of this change order. The City of Everett will do a <br /> complete overlay of Commercial Avenue,from Madison Avenue to Lowell Road,on the 2004 annual <br /> asphalt overlay project. The asphalt treated base placed on the pedestrian pathway along Commercial <br /> Avenue was placed to match the height of the existing pavement along Commercial Avenue. This <br /> essentially eliminated the placement of asphalt concrete placement class B along Commercial Avenue. <br /> Item#33—Asphalt Concrete Pavement,Class G.The City of Everett,again in an attempt to control <br /> project costs,eliminated the placement of asphalt concrete pavement,class G,on the pedestrian pathway in <br /> the Snohomish County PUD right-of-way north of Alta Drive to 44th street SE. <br /> Item#35—Cement Concrete Curb,Type E-4. Use of cement concrete curb,type E-4,was not used in <br /> the area where the trail leaves the north end of Commercial Avenue,resulting in a deduction of 185 linear <br /> feet of cement concrete curb from the contract. <br /> Item#36—Cement Concrete Curb&Gutter,Type A-1. An additional 15 linear feet of cement concrete <br /> curb&gutter,type A-1,were installed to complete work on the contract. <br /> Item#37—Cement Concrete Sidewalk. An additional 21.06 square yards of cement concrete sidewalk <br /> were installed to complete street restoration work on the contract. <br /> Item#38—Type A Thickened Edge for Sidewalk. The use of type A thickened edge for sidewalk was <br /> eliminated as a cost savings measure,resulting in the deletion of 230 linear feet of type A thickened edge <br /> for sidewalk on the contract_ <br /> Item#42—Extruded Asphalt Concrete Curb. An additional 173 linear feet of extruded asphalt concrete <br /> curb were installed beyond the 190 linear feet established in the bid proposal. This added curb was at the <br /> request of Snohomish County PUD. <br /> Item#43—Modular Block Wall. An additional 38 square feet of modular block wall were installed as <br /> part of the modular block wall at the Snohomish County PUD substation. <br /> Item#45—Control Density Fill. The 15 cubic yards of control density fill allowed for in the bid proposal <br /> were not used during construction on the project. <br /> Item#47—Bollards. Three additional bollards were installed at the request of Snohomish County PUD. <br /> Item#50—Chain Link Fence,Type 3. An additional 148 linear feet of chain link fence,type 3 was <br /> added to the contract at the request of both the City of Everett and Snohomish County PUD. <br /> Item#51 —End,Corner&Pull Post for Chain Link Fence. Thirteen additional end,corner&pull posts <br /> for chain link fence,type 3 were added to the contract by the City of Everett and Snohomish County PUD. <br /> Item#52—Double 14 ft.Chain Link Gate. One additional double 14-foot chain link gate was added to <br /> the contract,by the City of Everett,and Snohomish County PUD. <br /> Item#53—Relocate Existing Fence. This change order deducts$113.94 of force account bid item as not <br /> all dollars were needed to complete work under this bid item. <br /> Item#58—Topsoil,Type C. One cubic yard of topsoil,type C,are being deducted from the contract <br /> amount as it was not used on the project. <br /> Item#60—Hydroseeding. Approximately 2,200 square yards of disturbed earth will require hydroseeding <br /> to facilitate ground cover growth and prevent soil erosion. The City of Everett and KLB Construction Inc. <br /> renegotiated the unit bid price of hydroseeding on those quantities in excess of 125%of the original bid <br /> proposal,(750 SY),at the revised unit price of$2.50 per square yard. <br /> Exhibit A <br /> Page 8 of 10 38 <br />
