Effective August 6, 1999,the Agencies and the Consultant entered into'Contract#T00849T <br /> ("Contract")for the provision of consultant services to address technical issues related to_the <br /> evaluation of proposals for a Regional Fare Coordination project incorporating smart card <br /> technology. <br /> B. Effective March,2000,the Agencies and the Consultant executed Change Order No. 1 to the <br /> Contract adding Tasks 6, 7 and 8 to the scope of work. <br /> C. Effective January,2001, the Agencies and the Consultant executed Change.Order No.2 to the <br /> Contract adding technical analysis and King County project management services in support of <br /> the preparation and analysis of the Request for Revised Proposals. <br /> D. Effective August, 2001,the Agencies and the Consultant executed Change Order No. 3 to the <br /> Contract adding technical analysis of Best and Final Offers Proposals,project management <br /> support, and development of a smart card conversion plan for the State of Washington <br /> Departments of Social and Health Services and Employment Security, <br /> E. Effective July, 2002, the Agencies and the Consultant executed Change Order No.4.to reflect <br /> further schedule extensions and additional tasks necessary to complete the Phase III system <br /> procurement process. <br /> • <br /> Change Order No. 8 <br /> 3A Oct.25,2004 <br />
