•• <br /> EXHIBIT A 6 <br /> • <br /> SCOPE OF WORK <br /> Prepare and submit a monthly project status report to the City. Each monthly report will <br /> include schedule status,progress payment and budget status,project photos, change order <br /> status and potential problems or claims descriptions. <br /> 1.14 On-Site Construction Observation <br /> Provide full-time on-site specialty construction observation services to the City with <br /> supplemental inspections as required to address special needs. For budgeting purposes, <br /> field observation time has been estimated based on the stages of work as identified in the <br /> construction sequencing plans. <br /> It is anticipated that TIC's on-site Project Manager will perform many of the inspection <br /> activities personally. For those areas outside his expertise, additional inspection will be <br /> provided by specialists from the TIC Team. The on-site Project Manager will provide <br /> daily monitoring of the construction work. The representative will prepare written <br /> reports on the construction activities at the site,maintain a diary of activities,decisions, <br /> discussions with the contractor and other observations, conduct periodic on-site meetings <br /> with the contractor,document the pre-construction conditions and construction work by <br /> photograph, coordinate the delivery of any materials or equipment to be delivered to the <br /> City, and other work as assigned by TIC. Daily reports prepared by the on-site <br /> representative will be submitted to the City on a weekly basis. <br /> 1.15 Material Testing& Special Inspections <br /> Coordinate with the contractor all required material testing. Maintain a testing schedule <br /> that will inventory and track all required on-site and off-site material testing for the <br /> project. Coordinate retesting for all tests that fall to meet the specified requirements or <br /> are incomplete. Review contractor's selection and use of an independent testing agency, <br /> the frequency of required tests and documentation. <br /> Coordinate and provide special inspection services as required by the building code and <br /> the City. Coordinate with contractor to ensure coverage and timing of special <br /> inspections. Special inspection reports shall be submitted and reviewed, and copies <br /> provided to the City. <br /> 1.16 Operation and Maintenance Manual Preparation <br /> Assemble a draft operation and maintenance (O&M) manual for the project that is clear, <br /> concise and functional. Manuals will include a narrative of the pump station, tanks and <br /> piping operations, define operating conditions, and provide manufacturers' literature <br /> identifying all installation, operation, maintenance, handling, storage, assembly and other <br /> pertinent equipment information for all equipment, systems, subsystems, appliances, <br /> materials, finishes and other material furnished and/or installed on the project. <br /> It is anticipated that the draft operation and maintenance manual will be submitted to the <br /> City for review and comment eight months after the contractor is given notice to proceed <br /> in order to meet the schedule for the start-up of the pump station improvements and West <br /> Reservoir. TIC's Team will meet with City to review draft O&M manual and revise as <br /> necessary based on review comments. Provide an electronic copy of the final O&M <br /> Page 15of19 33 <br />
