Site activities involving ha•rardous or potentially ha•rardous materials shall bc conducted <br />in accordance with Contractor's sitc-spcciGc hcalth and safety plan. <br />B. Designatc a qualified representative as Health and Safety O�cer whose respnnsibility <br />will be health and safety monitoring and oversight. The designated qualified health and <br />safety representative shall be onsite at all times when contact with hazardous materials is <br />anticipatcd. <br />C. Contractor shall be responsible for providing safety training and shall require its <br />subcontractors and all Contractor-authorized visitors to have this training, if appropriate <br />for the work to be conducted by these pc�sonnel. Documentation of this training shall bc <br />availablc at the sitc. Provide appropriate personal protective equipment for Contractor's <br />employees, as specified in thc hcalth and safcty plan, and require subcontractors to <br />provide this equipment t'or subcontractor's employecs. <br />D. Provide for decontamination of Contractoe's and subcontractor's personnel and equipmem <br />that contact hazardous or potentially hazardous materials, in conformance with the <br />rcquirements o}'thc health and safety plan. <br />E. Provide for thc proper disposal of disposable safcty gear and equipment used by <br />Contractor's employees, the Port, federal and state agcncy representatives, and all site <br />visitors. Such disposal shall conform to al: applicable federal and local hazardous waste <br />disposal rcgulations. Waste material from Contractor's onsite decontamination facilities <br />shall bc properly containerizcd, labcled, and dis�osed of by Contractor. Rinse tvater may <br />bc Jrained onto sile soil wilhin arcas dcsignated by the Port, but shall not be dischazged <br />to storni se�vcrs. Disposal of rinse water via lhe sanitary scwer requires �vritten approval <br />by thc City of L'verett Department of Public Works. <br />F. Maintain accuralc accident and injuq� reporis and fumish the Pori a copy of the reporls <br />within 24 hours of the reported incident. <br />G.
