Diesel Retrofit Grant Agreement No. G0700022 <br /> between the Washington State Department of Ecology and City of Everett <br /> 2. Payments to the RECIPIENT from the DEPARTMENT shall be made on a cost- <br /> reimbursable basis. Payment requests shall not be submitted to the DEPARTMENT <br /> until the approved retrofit technologies have all been installed at the RECIPIENT's site. <br /> The DEPARTMENT's Project Contact shall require the Contractors to submit regular <br /> work orders, status reports, and completion statements to ensure efficient and timely <br /> use of funds. <br /> 3. Payments submitted to the DEPARTMENT Fiscal Contact using the DEPARTMENT's <br /> A19-1A Invoice Voucher shall be made on a cost-reimbursable basis within 30 days <br /> from the date the RECIPIENT's properly completed A1 9-1A Invoice Voucher is <br /> submitted to the DEPARTMENT. A19-1 As Invoice Voucher amounts and detail must <br /> coincide with work orders, status reports and completion statements in order to be <br /> reimbursed. <br /> 4. For the administration of this agreement the RECIPIENT shall follow the "Administrative <br /> Requirements for DEPARTMENT Grants and Loans", current edition. <br /> ( http://www.ecy.wa.gov/biblio/9118.html ) <br /> 5. Expenditures shall be monitored by the DEPARTMENT Fiscal Office for compliance with <br /> the EXPENDITURE BUDGET (listed below) at the PROJECT LEVEL. <br /> MAXIMUM GRANT <br /> ELEMENTS AMOUNT <br /> Task 1 — Coordinate Installation of Retrofit Technology $64,500 <br /> Total $64,500 <br /> PART 4. SCOPE OF WORK <br /> Task 1. Coordinate Installation of Retrofit Technology with DEPARTMENT-Selected <br /> Contractor. The DEPARTMENT shall specify a contractor to do the work required under <br /> this scope of work,and the RECIPIENT must use the specified contractor. The contractor <br /> shall provide the retrofit equipment and install the equipment at an agreed price resulting from <br /> a statewide bidding process. For transit authorities that choose to install diesel particulate <br /> filters,there may be different contractors associated with the different types of filters. In this <br /> case,the RECIPIENT and ECOLOGY will agree on which contractor is used. <br /> The RECIPIENT shall: <br /> • Designate an individual who has the signatory authority to approve contractor work <br /> orders, and verify and certify to the DEPARTMENT that the retrofit project for their <br /> fleet has been installed. <br /> • Schedule with the contractor, and notify the DEPARTMENT of the date for initial <br /> retrofit fleet inspection. <br /> • Provide written approval for the work order specifying the vehicles and retrofit <br /> equipment to be installed (the DEPARTMENT must also provide written approval <br /> 8 of work orders before installation can occur). <br /> • Schedule with the contractor for installation of equipment per the DEPARTMENT <br /> Page 3 of 6 <br />
