i <br /> � <br /> ( 1 DIVtNG SAFETY PLAN <br /> I 2 The Contractor shall abide by WAC 296-37 — "Standards for Commercial Diving <br /> 3 Operations"— as weli as the guidelines contained herein. The Contractor shall give the <br /> 4 Engineer 24 hours advance natice of any planned diving activity. <br /> I 5 <br /> 6 General Requirements— Communication and Safety <br /> 7 The following safety rules shall be followed whenever diving activity is perfortned. <br /> I 8 <br /> 9 • Prior to diving activity, the Contractor shall notify and gain approval from the <br /> 10 Engineer. Notification shall be made no less than one hour prior to the diver <br /> I 11 entering the water. <br /> 12 . Communica`ions between the diver and the Diver's Tender shall be maintained <br /> 13 at all times. <br /> 14 • The Enyineer must be notified at the completion oi oiving activity each day. <br /> � 15 <br /> 16 <br /> 17 LUMP SUM ITEMS <br /> I18 The Contractor shall submit a complete breakdown of all lump sum bid items showing <br /> 19 the quantity and dollar value assigned to each part of the work. Upon acceptance of the <br /> I20 complete breakdowns o( the various lump sum prices by the Engineer, they shall be <br /> , 21 used as the basis for all requests for complete payment and partial payment of lump sum <br /> 22 items. The Contractor is advised that payment for work under any of the lump sum price <br /> � 23 �trms will not be made until the Contractor's complete breakdown has been approved by <br /> 24 ihe Engineer. <br /> 25 <br /> I 26 Partial payment for lump sum items may be made monthly for items with a lump sum <br /> 27 price exceeding $2,500. The Co�tractor shall submit for approval a partial payment <br /> 28 breakdown of the lump sum item in easy to read tabulation (orm no later than 30 days <br /> I 29 prior to ihe first progress estimate cut-off date for payment of the individual item. The <br /> 30 Engineer reserves the right to direct adjustments within the partial payment breakdown <br /> 31 and to retain all authority in the method of partiai proc�ress payments. <br /> 32 <br /> � 33 The contract partial payrtient breakdown shall assign a percentage to each element of <br /> 34 material, work and direct and indirect costs. The elements shall total to 100 percent of <br /> 35 the lump sum price. The partial payment breakdown shall include and not be limited to <br /> � 36 the following: <br /> 37 <br /> 38 a. Percentages for the estimated materials costs of the major individual elements <br /> � 39 within each lump sum item. <br /> 40 <br /> 41 b. Percentages for the estimated labor and equipment costs to install the major <br /> , 42 individuai elements within each lump sum item. <br /> � 43 <br /> 44 c.
