EXHIBIT A <br /> 5 SCOPE OF WORK <br /> 10. The owner will be expected to provide access to all locked or restricted <br /> areas. The owner must provide an Escort to accompany the Building <br /> Inspector. Multiple samples of similar suspected ACMs will be collected <br /> to provide improved accuracy and quality assurance. <br /> 11. All bulk asbestos samples will be collected and transported under Chain-of <br /> Custody to a laboratory accredited under the National Voluntarily <br /> Laboratory Accredited Program (NVLAP) which is administered by U.S. <br /> National Institute of Standards and Technology. <br /> 12. All bulk samples submitted for testing will be analyzed for asbestos <br /> content by polarized light microscopy(PLM). <br /> 13. A report compiled and signed by an AHERA accredited inspector. Report <br /> will classify the types of ACMs, identify the sample locations through <br /> written descriptions, and will include approximate measurements of the <br /> identified ACMs based on the laboratory test results. Quantities of each <br /> ACM will be estimated. However, it is the architect's or abatement <br /> contractor's responsibility to confirm the actual square footage of ACMs, <br /> as this Building Inspector Report is not intended to be used as the sole <br /> basis for obtaining asbestos abatement bids. <br /> Phase Two: Asbestos Abatement <br /> The services to be provided will consist of the following: <br /> 1. Asbestos Abatement Specifications and "Bid Sheet Form" Design <br /> 2. An Asbestos Abatement Design will be developed by an AAHERA project <br /> Designer whom is also a Registered Professional Engineer with extensive <br /> experience in Asbestos Abatement Engineering Design. This Engineering effort <br /> shall result in the Development of "Specification Section 02850 — Asbestos <br /> Abatement" for insertion into the Architectural renovation specifications package. <br /> This will also include a Budgetary Abatement cost estimate and define the <br /> required time necessary for the Asbestos Abatement efforts by a Washington State <br /> Certified Abatement Contractor. <br /> 3. ALKAI will be available for "Walk-Through" to answer any questions by <br /> the Abatement Contractors related to asbestos prior to Bid Date. <br /> Page 11 of 14 <br />
