6.1.2. Services provided by the Advocate shall meet the <br /> requirements set forth by Washington State RCW 70.123 and <br /> RCW 70.125, and by the standards defined by the State of <br /> Washington Sexual Abuse/Assault Services Standards, Fiscal <br /> Year 2006. <br /> 6.1.3. Contractor shall participate in enhancing the Snohomish <br /> County Coordinated Community Response Team to increase the <br /> safety of victims of domestic violence,sexual assault, or stalking <br /> and hold offenders accountable. <br /> 6.2 Reporting. The Contractor shall submit data quarterly in InfoNet, <br /> relative to the provision of services. Reports date is due in InfoNet no <br /> later than January 15, 2008; April 15, 2008; July 15, 2008 and October <br /> 15, 2008. <br /> Contractor shall submit non-personally identifying demographic, service <br /> and compliance data required by state and federal funding sources in the <br /> InfoNet data collection system. The Contractor shall maintain <br /> documentation and records that support the data reported in InfoNet. <br /> Non-personally identifying client demographic, service and compliance <br /> data shall be entered into, and updated in, InfoNet at least quarterly <br /> beginning with clients on whom an intake is generated on October 1, <br /> 2007 and thereafter. First quarter data (October 1, 2007 — December 31, <br /> 2007) shall be collected on all new client intakes as of October 1, 2007 <br /> and entered into InfoNet as soon as possible after the end of the first <br /> quarter, but no later than January 15, 2008. <br /> After the end of the first quarter, for clients served during the period <br /> October 1, 2007 —September 30, 2008, all non-personally identifying <br /> demographic, service and compliance data shall be entered into InfoNet <br /> no later than October 15, 2008. <br /> The Contractor shall establish and maintain written procedures for the <br /> security of InfoNet use at its site. Procedures shall include: <br /> • Only authorized staff are provided access to the InfoNet data and <br /> files. <br /> • Staff are informed of the need for security and confidentiality of data <br /> and files maintained in or available through the InfoNet system; and <br /> • That the Contractor shall notify the Department Program <br /> Coordinator for this contract when an employee is no longer <br /> authorized to access the InfoNet system. <br /> 6.3 Acknowledgement of Federal Funds. Contractor agrees that any <br /> publications (written, visual or sound), but excluding press releases, <br /> newsletters and issue analyses issued by the City describing programs or <br /> projects funded in whole or in part with funds under the Agreement, shall <br /> contain the following statements. <br /> AGREEMENT RE STOP GRANT Page 3 <br />
