12 <br /> Consultant Services <br /> 1. Prepare draft and final permit applications and supporting submittal information <br /> (transmittal letters, project narrative, drawings) in a format that is acceptable to the <br /> permitting agencies. <br /> 2. Submit applications on behalf of the City. <br /> 3. Provide follow-up coordination support (e.g., responding to questions and requests for <br /> additional information) after permit application submittal. <br /> City Responsibilities <br /> 1. Review draft permit applications. <br /> 2. Provide all real estate information (legal descriptions, mailings for adjacent property <br /> owners, etc.) as required in the application process. <br /> 3. Pay all permit application and review fees, as well as newspaper notice fees. <br /> 4. Procure and post all notice boards required in the permit process. <br /> Assumptions <br /> 1. It is anticipated that the following permits/approvals will be required: <br /> o U.S. Army Corps of Engineers <br /> • Section 404 Permit—Nationwide Permit 12 Utility Lines <br /> o Washington State Department of Ecology <br /> • 401 Water Quality Certification <br /> • NPDES Stormwater Construction General Permit and Stormwater Pollution <br /> Prevention Plan (SWPPP) <br /> o Washington State Department of Fish and Wildlife <br /> • Hydraulic Project Approval (if stream(s) are identified) <br /> o Washington Department of Archaeology and Historic Preservation <br /> • Section 106 Compliance <br /> o Snohomish County <br /> • Grading Permit (including Critical Areas)/Targeted Drainage Plan <br /> • Right-of-Way Permit (D8 Utilities) <br /> 2. The consultant will submit applications on behalf of the City. <br /> 3. No public hearings will be required. <br /> 4. Haul Route Agreement and Road Closure Permit (if needed) will be obtained by the <br /> selected contractor. <br /> 5. Public hearings are not required for any of the permitting processes. <br /> 6. One Joint Aquatic Resources Permit Application (JARPA) will be prepared for: Corps <br /> Section 404 Permit, HPA, and Water Quality Certification. <br /> 7. The two Snohomish County permit applications will require an intake appointment (one J I <br /> appointment is assumed).
