2 943-15:5 <br />March 2} 1994 - <br />16-94, Sheets A1 and A2. <br />Target Surveyors, Inc. titled'Boundary & Topographic Survey Everett Care <br />Center for Nursing Home, inc., dated 48-93, �heet 1 of 1. <br />We understand that preliminary development plana include construction of an approximately <br />50,000 square foot nursing home (including basement level), and associated utility <br />improvements, and parking lot. We expect the building foundation loads will be moderate. <br />The building will be constructed of structural steel and concrete. A daylight basement level is <br />planned for the west portion of the building. <br />A member of our staff visited the site on March 4 and 8, 1994. The site was wedge shaped <br />and about 2 acres in size (Figure 2). The site was occupied by a one atory, wood frame, single <br />family residence, two storage sheds and a driveway. The aite topogrephy was relatively level <br />an the west half and sloped gently down on the east half. The site vegetation on the west <br />portion of the site consisted prim�rily of dense brush and mixed trees. The eastem portion of <br />the site contained primarily grass lawn with scattered cedar trees. <br />2. PREVIOUS INVESTIGATIONS <br />We are not aware of any previous environmental or geotechnical reports on the subject site. <br />3. RESULTS OF LL�VEL I BNVIRONMENTAL ASSESSMENT <br />The purpose of the Level I Environmental Assessment was to identify land uses, ownerships, <br />or activities that may have resulted in the generation, atoroge, use, or release of haz..rdous <br />materials or wast�c on the subject property. As requested, we contracted an asbestos testing <br />firm to conduct a survey of the existing house on the aite. Chemical sampling and analytical <br />testing of site soils and groundwatet wete not part of the scape of work for this Level I <br />[:nvironmental Assessment. In conducting the Level I Environmental Assessment we: <br />. Performed a visual survey of the site and vicinity; <br />. Reviewed available aerial photographs of the site dating back to 1947; <br />. Reviewed selected topographic and geologic maps of the site; <br />. Reviewed the U.S. Environmental Prctection Agency (EPA) Comprehensive <br />Environmental Response, Compensation, and Liability Information System <br />(CIRCLiS) list of known potentially hazardous sites; <br />. Reviewed the Washington State Department of Ecology's Leaking <br />Underground S�orage Tank (LUS'n, and Underground Storage Tank (US'n lists <br />for UST or LusCs in the project area. <br />. Reviewed the Washington State Department of Ecology (WDOE) Affected <br />Media and Contaminants Report, formerly the Hazardous Waste Investigation <br />Golder Assoclates <br />
