March ?A. 1994 — _ 6 943-1525 <br />Mary Meriam Mukilteo Water and Sewer District <br />Water service was hooked up to the house in June of 1959. The water district <br />currently purchases its water from the City of Everett. Everett's water comes from the <br />Spada Lake reservoir. No water quality problems have been detected or reported. <br />Sewer service was availablr to the site but no records of the sewer being hooked up <br />were found. <br />jeff (last name withheld) Snohomish County Deaartment of Health <br />A permit for a septic system is on record with the health department. The permit was <br />dated August 19, 1959. The septic tank is located about ?A feet north of the house on <br />lot 35. The drainfield extends to the north of the tank. Records showed that the <br />septic system was functioning as of 1988. <br />�rank Marshall C� of Everett Fire Department <br />'Phe fire marshall indicated that no flre response to the subject site was on record. No <br />hazardous materials unit response was recorded for the subject site. <br />3.4 Agency and Recorde Review <br />3.4.1 CSRCLIS and Affected Med3a and Contaminants Iteport Review <br />We reviewed a copy of the U.S. EPA's CERCLIS list (run dated June 4, 1993). This list <br />contains sites the EPA has identified as having the potential to contain hazardo�s materials. <br />We also reviewed the Washington State Department of Ecology (WDOE) Affected Media and <br />Contaminants Report, formerly the Hazardous Waste Investigation Clean-Up Program <br />(HWICP) list of known potentially haurdous sites. The list was run dated October 13, 1943. <br />This list is a compilation of sites that are: <br />• on the National Priorities List - state and federol; <br />. state sites - confirtned hazardous substances sites; <br />• state sites - potential hazerdous substance sites; <br />• state sites - undergoing long-term monitoring; <br />• sites which have been cleaned up. <br />These databases were searched end any sites with the same zip code as the subject site were <br />flagged and located on a base map. Sites that fell within about a mile radius of the subject <br />site are listed in the following summary end appear on Figure 3. None of the listed sites <br />were within l/2 mile of the subject site. The subject site did not appear on the CERCLIS or <br />WDOE's lists. <br />The following section contains a summary of the listed sites within about a mile of the subject <br />site. The letter preceding the site location corresponds to the location identification on Figure <br />3. <br />Golder Assoclates <br />
