<br /> "Cement Concrete Pavement Alternate",by adding 1,400 square yards of Cement Concrete Pavement
<br /> Alternate, at the unit price of$38.00 per square yard,resulting in a$53,200.00 concrete price increase.
<br /> New schedule 5, for new work order 30025. This work order is being created to track the costs to
<br /> construct full-width street replacement and pavement overlay of existing roadways of upper and lower
<br /> Laurel Drive which were originally designed to have trench patch restoration in the areas of the sewer
<br /> replacement work. It became necessary to do full-width replacement and overlay of these streets due
<br /> to damage caused by the heavy construction traffic loads during the course of construction.
<br /> Bid Item#002—Surveying. Add bid item#002 entitled, "Surveying", to new work order 30025, at
<br /> the lump sum price of$2,590.00.
<br /> Bid Item#003 —Traffic Control Labor. Add bid item#003 entitled, "Traffic Control Labor",to new
<br /> work order 30025, for 400 hours of traffic control labor, at the unit price of$39.00/hour,resulting in a
<br /> $15,600.00 contract price increase.
<br /> Bid Item#004—Maintenance &Protection of Traffic Control. Add bid item#004 entitled,
<br /> "Maintenance &Protection of Traffic Control", to new work order 30025, at the lump sum amount of
<br /> $4,000.00.
<br /> Bid Item#006—Sawcut Pavement,Up to 6-inch thickness. Add bid item#006 entitled, "Sawcut
<br /> Pavement,Up to 6-inch thickness", to new work order 30025, for 1,000 linear feet of sawcut
<br /> pavement,up to 6-inches thickness, at the unit price of$3.00/if, resulting in a$3,000.00 contract price
<br /> increase.
<br /> Bid Item#054—Concrete Inlet. Add bid item#054 to new work order 30025, for 3 each concrete
<br /> inlets, at the unit price of$500.00/each, resulting in a$1,500.00 contract price increase.
<br /> Bid Item#056—Storm Drain Pipe, 8-inch SDR 35 PVC. Add bid item#056 to new work order
<br /> 30025, for 145 linear feet of storm drain pipe, 8-inch SDR 35 PVC, at the unit price of$42.00/if,
<br /> resulting in a$6,090.00 contract price increase.
<br /> Bid Item#058—Storm Drain Pipe, 8-inch Class 52 DI. Add bid item#058 to new work order
<br /> 30025, for 14 linear feet of stoiiii drain pipe, 8-inch Class 52 ductile iron pipe, at the unit price of
<br /> $50.00/linear feet, resulting in a$700.00 contract price increase.
<br /> Bid Item#066—Concrete Curb or Curb and Gutter. Add bid item #066 to new work order 30025,
<br /> for 65 linear feet of concrete curb or curb and gutter, at the unit price of$12.00/linear feet,resulting in
<br /> a$780.00 contract price increase.
<br /> Bid Item#073 —Force Account. Add bid item#073 to new work order 30025, for force account
<br /> work as directed by the engineer, in the amount of$6,000.00.
