Add bid item#084 to new work order 30025, for 1,500 square <br /> yards of geotextile fabric, at the unit price of$2.05/square yard, resulting in a$3,075.00 contract price <br /> increase. <br /> Bid Item#085—Overexcavation. Add bid item#085 to work order 30025, for 150 cubic yards of <br /> overexcavation, at the unit price of$24.00/cubic yard, resulting in a$3,600.00 contract price increase. <br /> Bid Item#088—Perforated Drain Pipe with Drain Rock. Add new bid item#088 to work order <br /> 30025, for 115 linear feet of perforated drain pipe with drain rock, at the unit price of$14.00/linear <br /> foot, resulting in a$1,610.00 contract price increase. <br /> Bid Item#089—Crushed Surfacing Base Course. Add bid item#089 entitled, "Crushed Surfacing <br /> Base Course"to new work order 30025, for 1,200 tons of crushed surfacing base course, at the new <br /> unit price of$22.50/ton, resulting in a$27,000.00 contract price increase. <br /> Bid Item#090-HMA Class 'A-inch PG 64-22. Add bid item#090 entitled"HMA Class %2-inch PG <br /> 64-22", to new work order 30025, for 430 tons of HMA %2-inch PG 64-22, at the new unit price of <br /> $85.00/ton, resulting in a $36,550.00 contract price increase. <br /> Net result of the above fifteen bid items under new schedule 5 results in a$122,895.00 contract price <br /> increase. The net effect of changes made to schedules 1 &4 and adding new schedule 5 to this <br /> contract results in a $352,415.00 contract cost increase. <br /> JUSTIFICATION: <br /> Schedule 1, work order 30021 <br /> Bid Item#003—Traffic Control Labor. Additional hours of traffic control labor have been used on <br /> this project due to added work and duration. <br /> Bid Item#065—Driveway&Alley Approach, 6-Inch Thick. Additional square yards of driveway <br /> & alley approach, 6-inch thick have been installed in areas that were not originally planned to have <br /> new 6-inch thick driveways. Some sidewalks were installed as 6-inch thick and paid for under this bid <br /> item, because residence park on the sidewalks due to roadway width being so narrow in this area of the <br /> City. <br /> Bid Item#066—Concrete Curb or Curb & Gutter. Additional linear feet of concrete curb or curb <br /> & gutter were required as the original plans intended to leave several areas of existing, relatively new <br /> curb and sidewalk on the project along Tulalip and Snohomish Avenues and on 34th Street. In order to <br /> more fully comply with ADA requirements related to cross-slopes of sidewalks and driveway, it was <br /> necessary to re-design the new street and curb elevations which conflicted with the existing sections of <br /> curb and sidewalk. <br /> Page5of8 44 <br />
