Exhibit A to the Professional Services Agreement l <br /> between The City of Everett and The Blueline Group,LLC for the <br /> Water"L"Improvements-Hewitt Ave(WO#UP3462) <br /> dated October 28,2011 <br /> Task 003 - GEOTECHNICAL EXPLORATIONS <br /> Blueline will contract with Associated Earth Sciences, Inc (AESI) for Geotechnical Explorations. This <br /> will include: <br /> • AESI will be on-site during potholing (performed by the City) to obtain soil samples and test <br /> for corrosion as well as provide recommendations for use as backfill. <br /> • AESI will provide a letter that provides the results of their soil samples, including corrosion <br /> testing and recommendations (backfill, mitigation of unsuitable soils, excavation and <br /> temporary slopes, erosion control, and dewatering as needed). <br /> Assumptions: This scope does not include testing for contamination, vactoring, core <br /> samples, traffic control or signage (to be provided by the City), ROW <br /> permitting, piezometers. <br /> Deliverables: Electronic Word file of draft letter for review by the City. <br /> Electronic Word file, 2 hard copies, and 1.electronic copy of final letter. <br /> Task 004- PRELIMINARY DESIGN <br /> Using the base maps prepared in Task 002, Blueline will provide the City with Preliminary Design <br /> recommendations, plans, and Engineer's Opinion of Probable Construction Cost for the Project, <br /> including: <br /> • Review available survey information and conduct site visits as necessary to determine the <br /> most efficient, cost effective alignment. <br /> • Layout approximately 2,200 lineal feet of new 12-inch ductile iron water mains, including fire <br /> hydrants, replacement of water services up to the meter (nothing past the meter), and <br /> connections to the existing system. <br /> • Prepare Preliminary Plan sheets for the proposed water main improvements, including: <br /> o Horizontal alignment of water design shown in plan view only at this stage. <br /> o Preliminary Connection Details. <br /> o Sheets at 22"x34"with roughly an 18"x28" drawing area. <br /> o Drawing scale at 1"=20' horizontal and 1"=5' vertical. <br /> o Utilize City CAD Standards and coordinate with the City CAD Manager as necessary. <br /> • Prepare an outline Engineer's Opinion of Probable Construction Cost for the Project including <br /> quantities and a large contingency at this stage. <br /> Deliverables: Preliminary Design Plans (6 hard copies sets at 22"x34"). <br /> Preliminary Design Plans (PDFs and CAD - DWG &DWF files). <br /> Email accompanying the submittal that outlines assumptions, questions, and <br /> recommendations (Email). <br /> 1§_ <br />
