City of Everett ! March 12,2012 Page 7 <br /> 5 <br /> We will work with the City to determine the need for a USACE permit early in the project. This <br /> SOS and budget assumes that a USACE permit will not be required. The following would need <br /> to be prepared to support applying for a Corps permit: <br /> ■ Endangered Species Act review - may require a Biological Evaluation or Biological <br /> Assessment <br /> • Stream and Buffer Restoration Plan <br /> • Section 106 Cultural Resources-may require a Cultural Resources Survey <br /> ■ Ecology 401 Water Quality Certification-may require a Water Quality Protection Plan (WQPP) <br /> • Coastal Zone Management(CZM) Certification(Ecology) <br /> • Hydraulic Project Approval from Washington Department of Fish and Wildlife (WDFW): We <br /> understand the City has already engaged WDFW,and secured a Hydraulic Project Approval (HPA) <br /> authorization for previous construction of stream and bank stabilization measures within the <br /> project reach. We will work with the City to provide the project documents needed to apply for an <br /> amendment to the existing HPA to cover the proposed project. <br /> • Construction Stormwater General Permit (Ecology): This permit will be required if the project <br /> disturbs over one acre. If required this will be applied for during project design phase and <br /> transferred to the Contractor. A Stormwater Pollution Prevention Plan (SWPPP)will be required by <br /> Ecology. <br /> • Local and State Permits (Critical Area Compliance, grading permit, SEPA): The City will lead <br /> permitting for these items. GeoEngineers will provide support by providing copies of documents <br /> developed during Tasks 1 to 5. <br /> At the request of the City, GeoEngineers can prepare a supplemental proposal to assist the City with <br /> preparation of permit applications and supporting studies, such as completing a project biological <br /> assessment or other documentation that is determined to be required. <br /> Assumptions <br /> ■ City will complete all required site assessments and data gathering including: OHWM delineation, <br /> wetland delineation (if present), PHS inventory and assessment. <br /> • City will complete all local,state and federal permitting and pay all permit fees. GeoEngineers will <br /> • provide limited review and assistance. <br /> Deliverables <br /> ■ To be determined by City on as as-needed, on-call basis. <br /> TASK 7-SERVICES DURING CONSTRUCTION <br /> GeoEngineers will provide part-time construction observation and office support for stream stabilization <br /> measures to be constructed during the 2012 and possibly 2013. The purpose of the construction <br /> observation will be to assist the contractor with interpreting the contract documents, and observe that the <br /> project elements are constructed in general accordance with the design.
