The bidder shall 18 complete spaces on the proposal form that call for, but are not limited to, unit 19 prices; extensions; summations; the total bid amount; signatures; date; and, 20 where applicable, retail sales taxes and acknowledgment of addenda; the 21 bidder's name, address, telephone number, and signature; the bidder's 22 D/M/WBE commitment, if applicable; a State of Washington Contractor's 23 Registration Number; and a Business License Number, if applicable.
